6 I permit my ward to attend the offline classes in the school premises.
7 My ward has no any symptoms of COVID – 19
8 My ward will take all the precautions for COVID – 19 as directed.
9 My ward will follow the guidelines given by the school.
10 I will immediately inform the school if my ward develops any symptom of COVID – 19 like
cough, cold, fever or loss of smell and taste. I will also make sure to inform the school if
someone in my neighborhood is infected with COVID 19.
11 Despite of all precautions taken, if my ward develops infection of COVID – 19 I will not
hold the school or classmates responsible for it.
12 I am signing this self declaration cum consent letter after going through all the details
mentioned.

Guidelines for offline classes to be followed by students –

 Students must have mask/face cover and sanitizer with them.


 Students must follow all the COVID -19 protocols in the school premises.
 Students must not bring any mobile devices with them into the school campus.
 Students must get themselves checked with thermal scanner and sanitized at the school
entry gate.
 Social distancing must be followed at all times in the school campus.
 Students must bring their own drinking water and should not bring lunch box to the
school.
 Students must follow all the instructions given by school administration while in school
campus.
 Students of class X to XII must submit their Vaccination Certificate (1st Dose) to their
class teachers.
