Phase of Community Action Description and Purpose

Establishing links and partnership- Establishing links is a process of identifying


building with local groups and locating different actors and players
within a community who share a common
purpose, interests, and goals. Partnership-
building entails coalition formation and
strengthening relations with key community
groups, stakeholders, and individuals. The
purpose of it is to assess their knowledge,
perspectives, resources, and influences that
may be consequential in accomplishing
desires and community needs.

Community profiling Community profiling provides a descriptive


insight about the attributes and conditions of a
community. It involves the use of information
concerning the characteristics of a
community, the main purpose of it was to
helps community members be informed about
the physical, socio-political, and material
characteristics of their community.

Needs and resources management Need assessment used for examining the
cause and nature of the needs, as well as for
helping the community set its priorities for
future initiatives. Resource assessment helps
the community be informed about resources.
The main purpose of this is to fulfill the
community lacks and needs to acquire to
achieve its community goals and objectives.

Participatory action planning and Participatory action planning involves the


leadership development open and inclusive engagement of groups or
individuals in a community who have decided
to pursue a goal or solve a problem. The
purpose of it is to encourage participation,
such as those initiated by volunteer
community members provide opportunities
for community members to communicate with
each other and identify their problems and
draw solutions.

Resource mobilization Resource Mobilization process that deals with


the necessary steps in raising, maintaining,
and sustaining community support. The
purpose of Resource Mobilizations is to takes
into account the importance of people and the
strength behind their utilization of limited
resources.

Plan implementation Plan implementation refers to the phase where


actual and concrete implementation of
tasks and activities are done. The main
objective of plan implementation is to
accomplishment of the goals set by the
community, incurring the desired change.

Monitoring and evaluation Monitoring focuses on the status and


progress of tasks and activities in the course
of the implementation of the community
action plan. While evaluation is typically
conducted in the middle and at the end of the
scheduled completion of the community
action plan to evaluate the effectivity and
success of the project. The main goal of this is
to know the performance of project
implementers and the effect of resources in
completing tasks and activities.
Your class is tasked to formulate a community action plan using participatory approaches
for your chosen community.

Instruction:

1. The class will be divided into five groups.

2. Each group will formulate the part of the community action plan which will be
presented to the Office of the Community Outreach Program of the school for further
critiquing and approval.
