• V olproic acid exhibits its pharmocologic effects in a couple of ways -
1. Acts on GABA (Gamma AminoButyric Acid) levels in the CNS,
2. Blocks voltage-gated ion channels,
3. Inhibits histone deacetylase.
MECHANISM OF ACTION
• Impaired GABAergic inhibitory activity is pathophysiology of seizure initiation and propagation, given that controlling this
pathway a potential target for antiepileptic drugs. There is ample evidence that valproate increases turnover of GABA and
thereby potentiates GABAergic functions in some specific brain regions thought to be involved in the control of seizure
generation and propogation. GABA is synthesized from a-ketoglutarate through the tricarboxylic acid(TCA) cycle and
metabolized into succinate semioldehyde and then to succinate by GABA transaminase and succinote semialdehyde
dehydrogenase, respectively.
• Studies have shown that valproic acid inhibits GABA transaminase and succinate semialdehyde dehydrogenose, increasing
the GABA concentration by reducing its degradation.
• Valproic acid may also exert antiepileptic effects by reducing the high-frequency firing of neurons by voltage-gated sodium,
potassium , and calcium channel blockade. Valproic acid modulates the biochemical phenomenon of aura and affects
nociception by modulating GABA and/ or glutamate- mediated neurotransmission.
