NOTIFICATION FOR FEE STRUCTURE FOR ADMISSION IN ACADEMIC


SESSION 2021-22 IN M.B.B.S. COURSES.
The fee decided by the UP Government vide their letter no. 71-4099/34/2021-4-Part(2) /120293/2021
dated 08.12.2021, is as mentioned below:-

* Books and medical Insurance amount Rs 21000/- (to be deposited at the time of joining).

Note: 1. Hostel is compulsory.

2. It will be compulsory for the students to do internship from the College


itself.

3. Full Fee of the year will be deposited at the time of admission and before
the LAST DATE as per counseling notification. No Part payment of Fee is
permitted and Fee once deposited is non-refundable.

4. Student will pay all the tuition fees, hostel fee and other charges
approved by the admission fee regulatory committee every year between
1st February to 28th February failure which he will not be eligible to
appear in the university examination

DOCUMENT CHECKLIST
After submitting the original documents to the DGME, one set photocopies of the following documents must
be submitted at the time of reporting at the Mayo Institute of Medical Sciences, Barabanki.

1. Allotment letter issued by DGME U.P.


2. Admit Card issued by NEET
3. Result/Rank Letter issued by NEET
4. High School (10th ) School Certificate/Birth Certificate as proof of date or birth.
5. Senior Secondary School (10+2) Mark Sheet & Certificate
6. Identification Proof: Aadhar Card of Student, Father, Mother & Guardian
7. PAN Card of Student, Father & Mother.
8. Caste (OBC & SC/ST) Certificate (if applicable)
9. Domicile Certificate (if applicable)
10. Transfer/Migration Certificate
11. Character Certificate
12. Notarized study Gap Affidavit, if required (if study gap during between 12th standard to NEET admission)
13. Ten colored photograph of Student, which should be same as given in registration.
14. One -Passport size colored photograph of each of Father, Mother and Local guardian.
15. Ten -Self addressed envelopes (size: 5” × 10”) duly affixed postage stamps of Rs. 40/-on each
envelope
16. Four Post Dated A/c Payee Cheques in Favour of “Mayo Institute of Medical Sciences”, for second Year,
Third, Fourth & Final Year Fees.
17. DD/NEFT/RTGS in the name of “Mayo Institute of Medical Sciences” , Payable at Lucknow, Bank Name-
Punjab National Bank, Branch- Gomti Nagar, Lucknow, A/C No: 3926002105007490, IFSC Code:
PUNB0392600 ------------(for Hostel /Security /Miscellaneous /Other Fees)

Note: Demand draft of Tuition fee (Rs 11,21,162/-) will be given in the Favour of “ Director General,
Medical Education & Training, U.P. Payable at Lucknow at Nodal Centre.
