I have passed Intermediate level exam. I have registered for the Final New course or I will be registering for the Final New course on or before
01st May 2021 (OR) I have converted for the Final New course on or before 05th April, 2021 from earlier scheme. I have already completed my
articleship training or I am due to complete my articleship training before commencement of the exam or I am / will be in the last six or twelve
months of my articleship training as on 1st May 2021 and would be completing the same on or before the prescribed dates. I fulfill the eligibility
criteria for appearing in the the CA Final exam to be held in MAY 2021.
I understand that:
a) It is my duty to report errors/discrepancies, if any, in the details given above to the Examination Dept. of the Institute at the email ID or
address in the guidance notes.
b) I am aware that my admission to Final exam is based on this declaration and would be rendered invalid and void ab initio if the information
given above is found to be false at a later date. Such an act of furnishing false information would be deemed to be an act of unfair means, under
Regulation 41 of the CA Regulations 1988 and action initiated thereon which may include cancellation of the relevant result, debarring from
appearing in examination/s to be held in future, etc.
c) I accept that the decision of the Council of the Institute in this regard will be final and binding on me.
