1. Visit the Hazard Hunter website of the Department of Science and


Technology (DOST): https://hazardhunter.georisk.gov.ph/

2. Type in the Search Box the location of your agency/office/organization. See


sample below.

Note: If you do not have any agency/office/organization, you may put your
home address.

3. Double-click on the map the location to generate assessment. See sample
below.

4. Go to Assessment Results to see the hazards within your area. See sample
below.

5. Based on the results, accomplish the matrix in page 4 by copying the
assessment per hazard. See sample below.

6. Once accomplished, save the file and submit assignment via Training
Information Management System (IMS).
Upload the assignment using the filename template <surname> <assignment
number>
Example: (Dela Cruz_assignment 1)

Deadline is not later than 1600H of this day.
