BIOGRAPHICAL DATA
Via Malayao, the trainee was a natural born Filipino citizen was born at Brgy. San

Nicolas Bacoor City, Cavite on October 12, 1997. She is the third daughter of Leonila

Gajol Malayao- an Overseas Filipino Worker (OFW) in Beirut, Lebanon. She has an

elder brother named Daniel Gajol Malayao and an elder sister named Leda Gajol

Malayao.

Ms. Malayao finished her elementary education at Ligas I Elementary School in

Bacoor, Cavite. She finished her high school education at Bacoor National High School-

Molino Main located at Molino I Bacoor City, Cavite. She entered Cavite State

University Imus Campus for her college education wherein she studies Bachelor of

Science in Psychology. She is now on her 4th year in college and is now 20 years old.

SOUTHEAST ASIAN MEDICAL CENTER AND NATIONAL
CENTER FOR MENTAL HEALTH

Via Malayao

A narrative report submitted to the faculty of the Department of Social Sciences and
Humanities, Cavite State University, Imus City, Cavite in partial fulfillment of the
requirements for the degree of Bachelor of Science in Psychology. Prepared under the
supervision of Mary Grace L. Descarga, RPm.

INTRODUCTION

Anyone studying psychology will be all too aware that the repetitive and mostly

inaccurate replies you get can be enough to cause a psychotic breakdown themselves.

That inevitable response you get when you utter those fateful words “I’m a psychology

student”. “So what am I thinking?” hands down to the most frequent response you get

when people find out what you are studying and somehow studying psychology means

you must also be telepathic. And the thoughts in the psychology majors would be “Trust

me; I really don’t know what you’re thinking so yeah you are safe”.

Psychology is a very wide study field. There is a very specific process in

psychology and it cannot be ignored when studying a subject or a group of person.

Psychology is the study of humans mind and behaviors. Psychology is not only enclosed

on wearing a white scrub suit while testing maladaptive and psychotic behavior of clients.
Psychology can be in the fields of law enforcement, science labs, hospitals, animals,

healthy children and children with abnormal development, school, sports, society and

social interaction, and in design and technology fields. However, there are three major

settings that most universities focus on training the students.

The first setting is the educational. Interns are not allowed to teach here but to be

a helping hand in the Guidance Office. the works are mainly to attend to the students

problem I adjusting to the environment, students that are cannot be handled by their

teachers anymore, administering psychological test, interview and counseling to the

students, develop program and promote awareness to the students, and other psychology

related works.

The second setting is the industrial setting. The Human Resource Department is

the one who handles hiring process, testing, and salary of the employees. The interns here

are the one who will be human resource personnel and do the works in the said office.

and also, the Human Resource Department are the one who handles work related

concerns and conflicts also, the schedule of the companies extracurricular such as team

buildings that will enhance and boost the work performance of the employees.

The thirds setting is the clinical. Interns are allowed to observe, interact and to

give psychological assessments in the mentally troubled patients in any mental facility.

they are the ones who will make the psychological reports of the patients to help the

doctors know the patients behavior more.


Objectives

The general objective of the trainee is to learn and be educated in the setting and

put it into practice.

1. To be trained in the actual job with a direct supervision of a professional.

2. To expand the knowledge that the intern have in the actual job.

3. To improve her skills in handling normal or mentally troubled person.

Time and Place of the Practicum

The intern had her practicum in educational setting at St. Michael’s Institute from

August 15, 2017 – September 15, 2017, industrial setting at Southeast Asian Medical

Center from September 29, 2017 – October 27, 2017 and her clinical setting at National

Center for Mental Health from October 30, 2017 – December 8, 2017.

Significance of the Practicum

Actual experiences are the best teachers that a person can have in able to prepare

him/her in the real world. The intern needs experience to be able to be introduced in the

field of work she wants to be in and also to learn from her mistakes. According to John

Locke, our mind comes into this world as tabula rasa or “blank slate”. Therefore, all

knowledge comes from experiences and perceptions. A child should be taught virtue,

wisdom, breeding and learning in order to be competent individuals as they face the real

world.
EDUCATIONAL SETTING

History of St. Micahel’s Institute

The idea of establishing a catholic highschool originated from the mind of Fr.

Espiritu Hizon, priest of Bacoor from 151 to 1955. However, this was not realized

because he was transferred to another parish. Fr. Eliseo Perigrino took over his post from

1955-1958, and made the necessary preparation for the creation of the school. He met

some leaders of the parish and proposed the holding of fund raising project “Queen of

May”. The proceeds of this contest were used for the construction and purchase of the

furniture of the school.

In July 1956, St. Michael’s Institute formally opened its doors to fourteen

students. Mrs. Angelica Rito served as the principal with the following teachers: Fr.

Eliseo Perigrino, Religion; Mrs. Angelica Rito, English; Miss Virgilia Velasquez,

Science; Miss Visitaciondela Cruz, Math; Miss Lorenza Ybadlit, Filipino; Miss Felisa

Cuenca and Vocational; Miss Emilia Tortona later came as Filipino teacher.

In 1958, Fr. Peregrino was given a new assignment in the province of Bulacan. A

dynamic priest, Fr. Leon Coronel, assumed directorship of the school from 1958-1972. In

March 1960, the school had its first commencement exercises with 12 graduates. Violeta

G. Enriquez emerged as class valedictorian, Alice C. Pagunsan as salutatorian and Ernie

Alcova as first honorable mention. Meanwhile, new faces joined the faculty and several

teachers were hired. During the school year 1965-1966, Fr. Coronel introduced a new

administrative structure. He invited the Sisters of Saint Zita under the Congregation of

the Oblates of the Holy Spirit to administer the school. Sister Teresiana took over as
principal. The new religious administrators started the grade school department

organizing the first kindergarten class and adding a new grade every year. After two

years, Teresiana was replaced by Sister Maria Piera.

In 1967, typhoon Welming completely destroyed the school. Classes were

conducted inside the church so as not to interrupt the submitted school calendar. Fr. Leon

Coronel embarked on a new project, the construction of a new two-storey school

building. This new building was blessed on March 8, 1968 by his Excellency Msgr.

Artemio G. Casas, Bishop of Imus.

School year 1968-1969 marked another change. The Sisters of the Sacred Heart

took over the administration of the school. Sister Mary Emmanuel acted as principal

assisted by Sisters Mary Bernard and Mary Gertrude. Sister Maria Socorro was the

superior. It was during their supervision when the school had a rapid increase in

enrollment.

In 1972, Fr. Eugenio Marcelo became the next school director. Sister Mary

Teresita served as the new principal in 1974. She introduced a lot of educational

innovations and from then on St. Michael’s institute progressed in its educational

apostolate. Mr. Joseph Godio was assigned principal in 1978 after Sister Mary Teresita.

In 1980, Fr. Nicanor T. Victorino became the Director of the school as the

successor of Fr. Marcelo. He worked for the expansion of the high school building and

additional rooms for the elementary department. He also spearheaded the construction of

the gymnasium which became a venue for the different occasions not only the school but
also for the people of Bacoor. The late Dr. Rosalia Guinto was the school principal

known for her rich educational experiences and expertise.

She restructured the school’s academic program; supervised teachers in their

everyday tasks; handled discipline when it was necessary. All these were supported by

two Assistant Principal, Mrs. Rosalinda C. Mendoza in the elementary and Ms. Eugenia

B. Blanco in the high school department.

The founding of St. Peregrine Institute, gave way for Dr. Rosalia Guinto to resign

and eave St. Michael’s Institute in 1982. Fr. Victorino hired a new principal, a retired

public elementary school principal, Mrs. Arceli N. Ignacio from General Trias, Cavite.

She worked previously for the continuous improvement of the school. A Parents-

Teachers Association (PTA) was organized and Mrs. Ignacio supervised and supported

its projects. She gave Fr. Victorino an idea to put up the school canteen that was

supervised by Mrs. Pacencia M. Carmia. The income of the canteen that was became part

of the school’s financial stability. Mrs. Ignacio applied the school for the accreditation of

the Government Assistance to students and Teachers in Private Education (GASTPE), a

program of the government to subsidize the school fees of poor but deserving students.

There were more improvements, renovations, constructions and educational innovations

during her term. Added to theses, she continued her campaign for cleanliness and

beautification of the school.

After 10 years of her commitment as school principal, Mrs. Ignacio retired in

March 1992. In June 1992, Miss Lina V. Bernal, a former Pastorelle Sister of Jesus Good

Shepherd School in Imus became the new principal and severed for one year.
In 1993, Miss Eugenia B. Blanco, an exemplary Physics teacher who also served

as Assistant highschool Principal, was appointed as the new principal by the school

board. It was Mrs. Amelia C. Loba who became the Assistant Principal of the highschool

department while Mrs. Rosalinda C. Mendoza remained as the Assistant Principal in the

elementary department. Miss Blanco worked on the revision of the syllabi in all learning

areas, her term when our school became a member of the Cavite Diocese Parochial

Schools Association (CADIPSA) where teachers were sent to different seminars for their

professional development.

Miss Blanco also encouraged teachers to enroll in graduate schools for future

advancement in their teaching career. Her tasks and responsibilities grew deeper and

wider as she continuously and seriously performed them for the welfare of the entire

school community.

In June 6, 2003, in line with the celebration of his golden sacerdotal anniversary,

Fr. Victorino retired after 23 years of facing the challenges in his call of commitment as

Director of SMI and as the parish priest of St. Michael the Archangel Parish.

With the start of the SY 2003-2004, the school welcomed a new director, Rev. Fr.

Rosendo L. Ignacio Fr. Endong (as everyone fondly called him) was a tireless priest from

Sta. Maria, Bulacan but grew up here in Cavite. His plans for SMI favored both teachers

and students. He wanted to raise the standard of SMI to make it more competitive and

united under God’s love. He nurtured the heart and mind of everyone for love to the

Divine Mercy and to St. Maria Faustina Kowalksa. As he continued being the director of

SMI, the school continuously improved. Repairs, expansions, renovations, and re-
paintings of school facilities were made. Air condition units were installed in the

elementary rooms. The old elementary rooms in the 2 nd floor in front of the school gate

were renovated. Fr. Ignacio also initiated the Bible enthronement in the school and for the

teachers and students to lead, meditate and share the gospel before the first period of

mass in the morning.

He implemented the feeding program joined by Michaelians that was held once a

week in selected slum areas in Bacoor.

In School Year 2007, Fr. Ignacio conceptualized a three storey building to

accommodate the different offices, classrooms, and the other school facilities. Sadly, in

the midst of achieving this dream, Fr. Ignacio suffered kidney failure, and on March 27,

2007. He joined our creator for his final rest. After his untimely death Fr. George Morales

temporarily assumed the post and was appointed as OIC School Director from 2007-

2009. He continued the unfinished projects of Fr. Ignacio with Fr. Teodoro Bawalan as

the project engineer.

In 2008, the new 3-storey building was completed. It was temporarily blessed by

Fr. Morales so it can be used for the opening of the school year 2008-2009. In the same

year, the building was formally blessed by his Excellency Msgr. Luis Antonio G. Tagle,

Bishop of Imus now Archbishop of Manila.

More changes had been made to SMI to provide the students the best learning

environment. The chool library was relocated in the 2nd floor and became more

comfortable due to the installation of air conditioned unit.


Fr. Morales was replaced by Rev. Fr. Teodore in the year 2009 as the Director of

SMI. Fr. Bawalan has been performing tasks, being the parish priest of Our Lady of

Fatima Parish Church in Binakayan Kawit, Cavite. In the span of two years, the dusty,

rough and rocky ground of the school was cemented, and corridors were covered with

tiles. The old stage was demolished and replaced by collapsible structure for the plan to

construct a 4-storey building in preparation for the K to 12 Curriculum. In the year 2012-

2013, as part of CADIPSA unifying system, SMI started a new innovation for the

computerization of the pupils and student’s academic achievements, records and date

which was powered by Digital Information System and Technologies (DSAT).

In March 2003, the very hardworking and committed principal Miss Eugenia B.

Blanco retired. Among the applicants Mrs. Maria Lourdes Q. Nervaez, a very effective

highschool Math teacher, was chosen by the Board of Directors as the new principal. The

first achievement of Mrs. Nervaez, with the collaboration and cooperation of the entire

SMi community, was the FAPE-ESC accreditations.

On the 2nd year of Mrs. Nervaez as principal, SMI broadens its horizon with more

improvements. LED projectors were installed in every classroom in the high school

department, the implementation of E-Text ID system, the use of a new set of uniform for

the pre-elementary department and Grade VII, the installation of air condition unit in

Grade VII, the installation of air condition unit in Grade VII, Grade IX and Grade X

classrooms. In addition, SMI also required the ownership of Scuola degli Angeli which

was donated by generous owner. Mr. Zosimo Angeles. The aforementioned school is now

known as St. Michael’s Institute (Habay Branch) which currently offers Kinder to Grade

III with Mrs. Soledad S. San Jose as its Academic Coordinator.


In the School Year 2014-2015, an Administrative Council was, established to

promote better consultation and transparency on matters of school policies and

management. Mrs. Nervaez desires to achieve academic excellence through her

diplomatic and nurturing approach of involving the whole SMI community. She aims to

continue the legacy in making SMI a deacon of quality learning by giving the students a

truly evangelizing and transforming education thus preparing them to be more

competitive and agents of social transformation.

Mission

Inspired and guided by the Christ’s example, we the Michaelian’s, and quality

education that would promote total human development to form true Filipino Christians

who are makaDiyos, makatao, makabuhay, makabayan, and makakalikasan.

Vision

We, the St. Michael’s Institute, envision a community of transformative citizens.

Who are profoundly Christ-centered, socially oriented, promoters of justice and peace

and stewards of God’s Gift and resources.

Actual Observation and Experiences

Saint Michael’s Institute is a known private school in Bacoor City, Cavite for the

past 61 years. The intern is a student who came from a public school and being a part of

their faculty is a big adjustment for her even if it’s just temporary. There are many rules

that is strictly needs to be followed. One of these is students and even faculty members

are not allowed to use cellular phones in the school premises. This rule is difficult but
then good in many ways because you won’t be distracted while working or while

studying. You get to focus more in what you’re doing but then again the intern think it’s

too much for them to include the faculties too, given that they are on the right age to

control and regulate theirselves. The second one is that no one is allowed to wear skimpy

dresses, sleeveless shirts, ripped jeans and other revealing clothes. The intern wore a see

through blouse and 2 inches above the knee skirt which she think is wholesome and not

revealing but then given that her body frame is not that normal or tiny it is that

noticeable. But she still followed the rules so on her second day and the following days

are good.

The intern noticed that the ambiance of the school is solemn and peaceful because

it is a catholic school. The faculties and staffs will greets and smiles at the intern

everytime they cross paths. There is a prayer which can be heard throughout the school

premises lead by random students everyday. Novena and rosary are also present and

conducted by each department.

Duties and Responsibilities

The first week of the intern’s duty in the Saint Michael’s Institute is not eventful.

She got nothing to do because the files are still updated due to the visit that happened a

week before her first day. She got borrowed by the school registrar to do the unfinished

updating of students files and records. She arranged the files that include student’s school

forms, photocopied birth certificate, baptismal, and their parent’s marriage contract.

She’s able to arrange the students profiles from Nursery to Grade 12 all possible because

of the students master lists.


On her second week, she got to finally stay at the guidance office. She was tasked

by the School Psychometrician to interpret the results of the Performance Assessment of

Standards and Skills (PASS) of per domain results in the Science subject of the Grade 1

to Grade 10 students per section. The domains are Scientific Inquiry, Life Process and

Living Things, Materials and Their Properties, Physical Process, Earth and Space, Matter,

Force and Energy and lastly their Overall Performance.

Being an intern for her requires staying at your designated station so she’s able to

accommodate parents and students scheduled to talk to the Guidance Coordinator. And

she was also tasked to help for the paperworks after the counseling.

Problems Encountered

Lack of things to do, the working environment and the frequent celebration of

programs are the problems that the intern encountered in Saint Michael’s Institute. Due to

lack of things to do she got assigned in the registrar which is far to her main objective as

an intern striving to learn and get educated on the field. She’s done the paperworks that is

not really her job. Saying no to these things is hard for her especially when the superior

asked to do so. Although the working environment is solemn and peaceful, she’s not that

fine with the rituals that includes novena in english form and also regularly doing it. And

also due to programs, she’s obliged to watch programs instead of doing work loads.

Lastly, there’s this stereotyping that the intern experienced happened, because SMI is an

old school, expected that most of the teachers and staffs are old while there are few

young ones. The old ones labeled the new teachers including the intern as “Millenials”

which they got treated as kids whom different and more liberated than them. This
scenario is not okay for the intern because this results into groupings and hatred of the

millenials to the oldies.

Solution

Every problem has solutions but the best solution to the problems that the intern

encountered is to do what the superior asked you to do. Be friendly and try to socialize

more but not to the point that it will affect the work performance. Also,she adviced not to

mind the stereotypes and the groupings in the work environment because that is just a

normal scenario.

Insights

Working at the Guidance Office is not that eventful for the intern and she thought

this will bore her. Though the counseling part and interacting with the parents are the

ones she likes to do. The intern gets to know the students with problems and guide them

towards right and wrong actions to take. Paperworks are toxic for her especially if it is

loaded and all she got to do everyday is to interact with the papers than people. Even if

the intern didn’t like the actual job, she still able to execute her skills and do the best

work she can from the first day up to the last day of her internship.

The intern also thought that the worst problem she encountered was the students

thinking of the Guidance Office. She got to observe students reaction when they heard

about it. The student thinks that the Guidance Office is a room for detentions and

punishments to their wrong doings. The intern hopes that someday this will change and

the students will realize that Guidance Office can be their friend.
INDUSTRIAL SETTING

History of Southeast Asian Medical Center

Southeast Asian Medical Center (SAMC) is the first modern hospital in the

Southeastern side of Bacoor City, Cavite. The departments of the hospital are second

level. It was established since October 2009 and is started as Out-Patient Department. In

March 2013, the hospital was opened for operation and surgeries. Currently, they have

more than 200 direct employees.

Mission

Southeast Asian Medical Center, a modern medical facility is dedicated for the

delivery of quality, affordable and accessible holistic healthcare through medical

excellence and compassion.

Vision

To be the Premier Center in the region providing quality healthcare with

unparalleled medical expertise and amenities to meet the global challenges of health

delivery in a new world class hospital environment.

Actual Observation and Experiences

Southeast Asian Medical Center (SAMC) is a 7 storey medical facility with a bed

capacity of 50 patients. Two storey of SAMC which is 5 th and 6th floor are the wards

while the rest are for the offices. SAMC is still proposing the rest of the floors including

the 8th, 9th, and the 10th floor.


The intern is assigned in the Human Resource (HR) Department wherein she

handled different kind of office stuffs. The setting is medical and is expected to have a

clean environment. The intern thinks the office is quite small to be a HR office because

as every year passes, the hospital grows bigger and it needs to have a large office to

accommodate the applicants in the future.

The intern got the chance to interview applicants, be an usher for the kids during

the Halloween trick or treating and attend seminars which enables her to interact with the

employees. The intern thinks that her overall experience was good and fruitful.

Duties and Responsibilities

The intern arranged files such as employees’ profiles (201 files), contracts and

other Human Resource related documents. She also got the chance to interview some

applicants in the medical field. She interviewed registered nurses during her duty. At the

time of the cut off, she got the chance to give away the pay slips of the employees.

Problems Encountered

Due to the continuous resignation of the Human Resource Personnels in the

hospital, the files of their employees, resigned, and applicants are not updated. The

resume of the applicants from the very beginning that the hospital was built was still

there. She was tasked to send messages to those applicants even if the resumes are years

ago.

Also, another problem that the intern experienced was that other offices in the

said hospital were jealous because Human Resource department have 2 interns while they
don’t have. There’s this time that the intern was borrowed to do office works that is not

really her job. She got borrowed in the Credit and Collection department in which she

arranged billing files of the patients in the hospital.

Solution

Human Resource department files are now updated because the 2 intern manages

to arrange the files including the interview process and the resumes of the applicants in

the office field, medical fields or in the service field. The solution to the second problem

that the intern encountered was to just follow what the head of the department said so that

this will not be a cause of a conflict.

Insights

The intern learned that medical field and office field is different in terms of

interview and hiring process. The Human Resource department is responsible in hiring

employees in office field while in the medical field, the Human Resource Personnel is not

the one responsible for hiring the applicants but they’re just the first step of the hiring

process. There are different officials in the fields in the hospital that are responsible and

have the power to decide the hiring process. There is laboratory department head, nursing

services head, medical technologist head, pharmacy head, and others.


CLINICAL SETTING

History of National Center for Mental Health

The National Center for Mental Health was established in 1925 through Public

Works Act 3258. It was formally opened on December 17, 1928 and was originally called

the INSULAR PSYYCHOPATHIC HOSPITAL. It was later called the National Mental

Hospital. On November 12, 1986, it was renamed NATIONAL CENTER FOR

MENTAL HEALTH (NCMH) through the memorandum Circular No. 48 issued by the

Office of the President.

National Center for Mental Health (NCMH) is dedicated to delivering preventive,

curative and rehabilitative mental health care services. It is recognized as Special

Research Training Center and Hospital under Department of Health on January 30, 1987.

It has an authorized bed capacity of 4,200 inpatients and served an average of 56,000

outpatients per year. It has a total of 35 Pavilions/Cottages and 52 wards which sprawls

on a 46.7 hectare compound. It has its own medical library, sports center, chapel,

conference rooms, tennis and basketball courts, and ample parking spaces. A small

hospital within the hospital, the infirmary is equipped with x-ray and ultrasound facilities.

Philhealth accredited 100 beds and classified it as medical center. Rooms are always and

readily available throughout the year.

Actual Observation and Experiences

National Center for Mental Health (NCMH) is a medic nal facility

accommodating mentally trouble d patients in every cases and ages. There are almost 65

pavilions in the said hospital. These pavilions are their very own departments. The intern

was assigned in the Adult Female facility in which she gets to see the situation of the

patients in the hospital. The intern observed the facility in which she learned what is

mental hospital is in true life. She was shocked when she first saw a patient in episode in

the adult male facility. The patient was tied at the bed because he is screaming and trying

to dismantle the fabric tie. And also, the smell of every wards are weird in the interns

perspective.

Duties and Responsibilities

The intern was assigned in the Pavilion 3 which is the Adult Female facility. The

cases at this pavilion are mostly mood disorders and schizophrenia. She was first

introduced at the pavilion 3 and observed first the ambiance and the environment of the

wards. After she was comfortable administering test on her own she got her first patient

in the pavilion 6 pay wards. Pavilion 6 has the pavilion 1 and 3 extension on it. It was
more pleasing than those of the real pavilion 1 and 3. She got to administer the

psychological tests such: Bender Visual Motor Gestalt (BVMT) Test, Draw A Person

(DAP) Test, Weshcler Adult Intelligence Scale-Revised (WAIS-R), and Sach’s Sentence

Completion Test (SSCT). The intern will need the scores and interpretation of patients

test in order to make psychological report. She needs 2 psychological report that means

she needs 2 patients in order to complete her requirement to finish her internship.

Problems Encountered

The intern knew that it would be hard to interview and assess patients given that

they are mentally troubled. The moment she sat on the chair in the pavilion 3 with her

patient in her front, she didn’t expect it to be that hard. Her first patient is not cooperative

because she’s disturbed in thoughts. The intern didn’t get the chance to interview well the

patient to the point she even visit the patient three times because the second time she

visited her was she’s severely disturbed and she doesn’t want to be interviewed. Luckily,

the patient finished the tests at the third meeting of their assessment.

Solution

“Patience is a virtue” that’s what the intern thinks every interview she got to make

with her first patient. The only key in finishing the assessment is that she doesn’t give up

even if the patient is not that responsive and are disturbed in thoughts. And also, she is

willing to apply that phrase in everything she do.

Insights
National Center for Mental Health (NCMH) is a large facility for mentally

troubled patients. The intern learned so many things in this facility. First, is she learned

how to make a psychological report. Second is that she learned how it is to be in a mental

facility. She learned how to properly handle and conduct psychological assessment, how

to work handle the assessment when the patient is not cooperative and how to deal with

the mentally troubled patients.

It was the intern’s first day of duty. She got to meet the staffs which are all

friendly to her. The principal doesn’t like the outfit of the intern so she got to tell her to

wear something appropriate for a catholic school which the she obeyed the following

days of her duties in the said school. She got nothing to do in the Guidance Office so the

Guidance Coordinator told her to help out the registrar in arranging the students file. This

is what the intern’s task the whole day.

It was the intern’s second day. She wore pants and obeyed what the principal said.

She reported at the registrar still doing the papers she left the other day. She arranged the

students file each section.

The intern still worked at the registrar. She continued the files of the students’

records that she left the other day. She still has class at 2pm so she reported half day.

The intern met the school psychometrician. She told the intern to stay at the

Guidance Office. She gave the intern a task to do which is a psychology related work.

She didn’t able to finish the task because the task is too long and it covers the Grade 1-

Grade 10 students Performance Assessment of Standards and Skills (PASS) scores.

The intern accommodated the 2 parents whom had a concern in the Guidance

Office. The first parents concern is that his son (which is a Grade 9 student) said to be

caught gambling in their own classroom. The student got caught by the school librarian.

The parent said that it was not her son who did that because at that time, his son is

confined at a hospital so it’s impossible to happen. But due to the old offense that the

students commit which is a Public Display of Affection (PDA) with a Grade 10 student at

the school premises, the student need to do a community service as a punishment. The

second student is the one who actually gamble and the one who got caught in the act. He

had to do the community service as well.

The intern’s task is to still interpret the results of the Performance Assessment of

Standards and Skills (PASS) scores. She got invited to the presentation of the High

school students for the Buwan ng Wika program for the following day. She got invited as

well to play volleyball together with the millennial teachers but she refuse to join due to

the task she’s doing.

The intern’s day starts with the consult of a senior high school teacher in the

psychometrician. The teacher is bothered by her students drawing. She instructed her

Grade 12 students to draw a comic strip showing the Filipino culture and society. One of

her student draws a rape scene where the setting is an elementary school. The characters

are the woman pre-school teacher which is the victim and her student which is the

perpetrator. The story is detailed from the first time the student got interest with the

teacher up to the time he committed the crime. The last page was the most eye catching

part because the teacher is in a bare state and looks lifeless. The psychometrician together

with the intern assessed the drawing using a Draw A Person (DAP) test. The

interpretation shows that the student shows introverted trait, socially aloof and is trying to

hide her true self. They also checked the student profile only to find out that she’s living

with a guardian because her mother is working in manila while her father is dead. The

student also loves to watch gruesome, murder and surgical films.

the intern’s task was again to duty in the registrar office. She did the unfinished work she

left on her third day. She’s thankful she finished the task during that day.

The intern finished the interpretation of the students Performance Assessment of

Standards and Skills (PASS) scores in that day. After that, she was tasked to do the

students attendance sheet for parents orientation from Nursery to Grade 12 which is

almost 90 pages. The population of the students reflects on the number of the table

columns.

The intern printed the attendance sheet she made the last time. She got nothing to

do after that so she just searched for downloadable psychological tests in the computer in

the Guidance Office. She printed it and gave it to the psychometrician.

Tuesday September 5, 2017

The intern conducted brief career counseling in the Grade 9 St. Agnes students. In

able to make an interaction to the students, she asked them what their plans are after they

graduate. She gave them questionnaire in career counseling to have their answers

recorded.

Later that day, the intern guided the visitors of different schools for the career talk

and endorses their schools. The schools are college universities namely; Asia Pacific

College (APC), University of the East (UE), Philippine Maritime Merchant School
(PMMS) and Universal College-Paranaque (UC). The intern made the schedule of their

talks for organization. The visitors mainly focused on the Grade XII students because

they are the one who will graduate this year. The intern also arranged the accomplished

questionnaire on career counseling of the students and she compiled it in the boxes

assigned for their section.

The intern conducted a individual counseling to the boys of Grade 6 St. Claire.

The topic of the counseling was about the student’s personal and academic happenings.

Other students are not comfortable to talk to the intern but others are comfortable to open

up their personal.

The intern got to finish the individual counseling of Grade 6 St. Claire. This time

she handled the girls which seems to be easy for her because the girls are much

approachable than the boys.

Friday September 8, 2017

The intern helped in judging the preliminary audition for the contestants of the

Michaelian’s Got Talent. This is held at the St. Michael’s Institute’s Auditorium. She

became one of the judges. After this the intern goes back to the Guidance Office and

arranged some files and records of the students there.

At the morning, the intern observed and watched the students practice for

tomorrow’s sportfest. And at the afternoon, she was in the Guidance Office to

accommodate students and parents with concerns.

In that day, its St. Michael’s Institute’s sportfest and also the last day of the intern.

She got nothing to do so she just watched the table tennis tournament and the volleyball.

Later that day, she threw out a mini party before she goes home.

words are that are associated with the employees profiles. That’s all the intern done the

It is the interns second day and she done the arrangement of the Daily Time

Record (DTR) and Overtime slips of the employees. She arranged it according to their

department. After the lunch she again does the dividers for the 201 files. And also, the

intern goes to the Medical Records for the logo seal of important documents in the

Human Resource Department.

The intern’s day starts with the sending of text messages to the applicants. The

text message contains the schedule of the examination of the applicants, the time of the

examination and the date. The applicant needs to confirm his/her name in order to be

officially scheduled. But no applicants confirm text messages because the resume was

passed 1 to 4 years already. The intern just made the resume as a scratch paper and other

confidential files shredded. While the rest of the day, the intern arranged the 201 files of

the employees one by one and sort the contents of it for better arrangement.

On this day, the intern helped in the examinations of applicants. The examination

includes the brief personality test, brief interview test, information sheet and the nurse

exam/ medical technologist exam depending on their position. The intern also conduct

initial interview to the nursing applicants. She asked about their personal life, their

educational background, their experiences and asked them if they are aware of the

provincial rate so that they are much knowledgeable about their future salary if ever they

got to pass the requirements. And also, the intern only reported in the morning at this day

because she got something important to do.

At this day, the intern arranged the 201 files of the employees. She organized

their files one by one. She also arranged Daily Time Record and Overtime slips of the

employees. The intern’s day is generally more off arrangements of the records and files.

The intern goes into the purchasing department to photocopy some of the contracts of the
employees because the printer and Xerox machine in the Human Resource Department is

for short bondpapers only and the contract needs long bondpaper.

The intern and her co-intern distribute Personal Record Sheet (PRS) to update the

records of the employees whether they changed name due to marriage especially those

woman employees and also other changes such as change of address, contact numbers

and identification number in their government cards. She also does the filling of the 201

th at she left the other day. At the afternoon, she was instructed to go to the

administrative office located at the 7th floor of the hospital and got the paper signed by the

director of Southeast Asian Medical Center (SAMC).

The intern arranged the 201 files of the employees in the Southeast Asian Medical

Center (SAMC) the whole day of her duty. While at the afternoon, she was ordered to go

the Customer Care Service Department give some documents to Ms. Josie (the customer

care representative) and have those documents signed by her.

Monday October 9, 2017

The intern is borrowed by the Credit and Collection Department wherein she

arranged the billing of the patients in the past few years of the Southeast Asian Medical

Center (SAMC). She used the system of the hospital which is called Hospital Information

Management System (HIMS) and checked each of the patients whether they are fully
paid in the bills or not. She checked each of the patient’s record in any services that the

hospital offers.

The intern interviews some applicants in the medical field. She asked about the

applicant’s family background, educational background, the reasons of his/her sudden

change of workplace if resigned to the previous job, the distance from home to the

hospital, work experiences and about the rates of the hospital. At the afternoon, she then

arranged the 201 files of the employees.

The intern arranged the 201 files that she left the other day. She gathered the

Personal Record Sheets of the employees in each department who hasn’t return yet the

papers. She managed to put the sheets in the 201 files of each employee.

Friday October 13, 2017

The intern again arranged 201 files. She and her co-intern printed the names of

the departments. And have it glued outside the filling cabinet assigned to each

department. She shredded some confidential files that is no longer needed by the Human

Resource Department head.

The intern goes to each department for the memorandum of the upcoming trick or

treat event what will be on the 27 th day of October 2017, 3pm. The employees are

encouraged to let their children join the said event. Each employee can bring atleast 3
children. And also the memorandum of the non working holidays in the November due to

All Saints Day.

The intern got into a seminar called “Global Hand washing Day” in which she

watched performances by each department in the hospital. She learned about the

moments of hand washing and how to properly wash hands and when to wash hands. The

intern is allowed to join the seminar even if it is for employees only. After the seminar,

they gave away freebies such as pouch with an alcohol bottle sponsored by Dr. J and 2

ballpens from the hospital. And also, there’s a food and drinks for each employees

waiting at the exit of the conference room.

The intern finally finished the sorting and arranging of the 201 files that day. The

201 files are already arranged by department of the employees and arranged

alphabetically by their surnames each department. Also the two drawers are separated,

one of the drawers contains the office workers and the other one is for the medical

workers.

The intern shredded some confidential files of the employees and of the company

the shredder in the Human Resource Department. After that, she got nothing to do.

The last day of the intern is mainly helping the arrangement and preparation of the

Spooktacular or the hospitals celebration of the Halloween. She helped at the registration

booth for the children of the employees that are joining the trick or treat in every

department in different floors of the said hospital. Though, she also helped in the

distributing of the payslips of the employees before the said event. She called every

department through the telephone in the Human Resource Department.

The intern arrived at the mental facility at exactly 7:30 in the morning. This is her

orientation together with her Clinical Instructor (CI) Mrs. Henedina I. Tayag. Sadly for

her part, she didn’t able to meet Mrs. Tayag because she’s busy and she’s not present on

that day, but instead, Ms. Marian Jasmine A. Lopez explained every psychological tests

that is need in assessments. She is also the one who stood as clinical Instructor for the

interns in behalf of Mrs. Tayag. The tests explained are the Bender and Visual Motor

Gestalt Test (BVMGT), Draw A Person Test (DAPT), Sach’s Sentence Completion Test

(SSCT), and Weshcler Adult Intelligence Scale – Revised (WAIS-R).

In the morning, the intern observed in the Pavilion 3 which is the Acute Female

Adult facility. She made General Observation of the patient she observed while it is being

in the assessment. She observed also the facility and the environment inside it. She
noticed that the patients had contagious head lice. They are all covered by blue hairnets to

avoid the workers such as social workers and nurses to be infected. While at the

afternoon, she observed in the Pavilion 1 Acute Adult Male facility. She observed the

environment and noticed that there is a big difference in their wards compare to the

female. Male wards have strong smell than females. And also, she noticed that when the

patient is disturbed and is running amok, they are tied in the bed by their feet and hand to

control their wildness and anger. They are being isolated to other patients to avoid

conflict and riot.

At this day, the intern together with her partner co-intern got a patient wherein

they could practice and conduct assessment. They are assigned in Pavilion 6 in which it

has the Pavilion 1 and Pavilion 3 pay ward departments. The patient they handled is in

Pavilion 3 ward 2. The wards here are more clean and good than in the pavilion 3 non

pay ward. They got to meet the patient. Jane Doe was 37 of age and is mentally troubled

due to illegal drugs. She dances at stri club for her living but didn’t enumerate much

about what happened. She is mentally disturbed, disoriented in thoughts, and is lack of

affect display. They didn’t finish the assessment and is scheduled to a second assessment.

The interns returned for the second assessment of the patient but the patient was

discharged and is in infirmary because of her complain in her constipation. To spend the

time wisely, they got another Jane Doe who is 38 years old. The patient shows

cooperation and is focused in the assessment. She answers the questions accordingly but
is disturbed when the questions are about being married. They finished the assessment in

that day.

The interns finished their General Observations and had it checked by their

Clinical Instructor for few corrections and revisions.

They went back to the Acute Adult Female pay ward department for the second

assessment of the first patient they handled. The patient was more disturbed than the last

time and said she was sleepy. They didn’t able to finish the assessment again that’s why

they scheduled another assessment to the patient.

Monday November 20, 2017

At this day, the intern got another patient but this time, she’ll conduct it

individually. Jane Doe is a 48 years old woman and is cooperative in the assessment. She

answers the questions accordingly and shows interest in what they are doing. She shows

willingness to get well. They finished the assessment of the patient. The intern got one

psychological report to be made.

The intern finished the General Observation of the last patient she handled. They

went to the Acute Adult Female Department again to see the chart of the patient and get

some information that will help her to finish the psychological report.
Wednesday November 22, 2017

At this day, the intern together with her partner co-intern, are scheduled to finish

the assessment for the first patient they handled. Although the patient is disturbed, they

managed to finish the assessment that day because if not the psychologists will be the one

to finish the assessment and the intern should find another patient to assess. After they

finished the assessment, they photocopied the protocol (the assessment they made) and

passed the original to the Ms. Lopez.

The interns got no tasks to do so she continued her psychological report and her

General Observation of the patients she handled. She didn’t able to finish the

psychological report so she’ll just continue it at the next day of her duty.

The intern continued the psychological reports she’s making the other day. She’s

making the psychological report of her 48 years old patient. This is the psychological

report she’s going to pass first and be revised by her Clinical Instructor.

This day is just considered as a counted day because the psychologists had a 3 day

seminar from November 27- November 29, 2017. Although the intern counted it, it won’t

affect their total hours of duty because theirs was more than 120 hours. In that case, the

At this day, the intern got to meet her group mates in Group dynamics they’ll be

conducting next Friday. Her group mates were from Centro Escolar University – Makati.

They got along well and she was tasked to make the invocation and facilitator of her

suggested Group Dynamics Activity.

Suppose to be, it is the interns last day of duty. She already completed the 120

hours of internship but then she needs to still report because of the revision of her

psychological reports. And also, they still need to conduct Group Dynamics with the

patients because that is the requirement before they finish the internship. The intern

passed her one accomplished psychological report to Ms. Lopez.

At this day, the intern received the revision of her psychological report, Ms.

Lopez explained the revision on how to improve and correct the psychological report she

made. And also, they were said to pass the 2 psychological reports in Friday. And also,

they are asked to go to the NP Screening at Pavilion 2 Out-Patient Department and had

the papers signed by Mr. Constancio Paubsanan.

At this day, the intern printed out the two psychological reports she made and

have it passed to Ms. Lopez for it to be graded. After she passed the prych reports, she

goes to her groupmates in Group Dynamics for the preparation of their program. They go

to Pavilion 3 Ward 2 to present their Group Dynamics together with the selected 21

patients. At first, the intern led the prayer then another groupmate of her does the opening

remarks. After that they proceed with the Group Dynamics 1 which is the patients are

required to draw a face and write down the things theyre thinking at the inside of the face

then present it in the front, before they leave they are asked first to look at a box and

describe what are they seeing inside this (the box contains mirro in which the patients

will see and describe themselves). Second is they proceed with the energizers and the

second Group Dynamics activity in which the intern facilitated is the activity named

“Bata Banderitas”, the patients are asked and instructed to put their answers inside the

body of the human figure that the interns gave them. At the face, the patients will write

what are they feeling as of the moment; At the left hand, they will write their significant

others; At the right hand, they will write their dreams in life; At the left foot, they will

write down their wants; At the right foot, they will write down their needs and they will

write down what are they holding on to in the chest. Their Group Dynamucs ended with
them giving each patient a food and a souvenir (a pink towel). They thanked the patients,

doctor, and the attendants before they left the ward. Before they went home, they get their

evaluation sheet at the office.
