Simulation Reflective Assignment

The simulation activity I learned in class is the case of one elderly patient named Sherry (66

years old) with Diabetes Types 2 for 10 years. She is overweight and has blurred vision. She is

on a required nutrition diet but she confirmed that she is going to die. Therefore, she has not

accepted the diet and tends to eat chocolate many times a day. For me, it is one of the interesting

situations. It is also an opportunity for me to practice my communication skills during class.

The problem of Sherry is that she is addicted to chocolate while she has treated her Diabetes. In

this situation, my colleagues and I have to convince her and explain for her to not touch

chocolate anymore as well as support her spirit to continue her treatment in the hospital. Before

we approached the patient, we discussed how we would communicate with her. Some points that

we need to pay attention to are that she has blurred vision and because she is elderly, so she may

have problems with movement, etc. As I observed when my colleague talked with her. The first

thing that we do is like what we learned in class, we need to approach the patient slowly, do step

by step, do hand hygiene, knock on the door, privacy, and introduce ourselves. Then, we need to

ask for the consent of the patient. We need to ask about her status, about the light and make sure

that she is comfortable during the conversation. And then, we explained her situation to her and

encouraged her to follow the nutrition diet because her Diabetes does not allow her to eat food

that includes too much glucose. She said the chocolate on the table is her son gave her and she

wanted to eat them although she knew about her disease. My colleague talked with her about

replacing another candy but including less sugar. Or she can eat them with reducing following

the control of the doctor or her main care provider. I realized that she is not a grumpy patient and

she does not tend to get angry when we cannot allow her to eat chocolate. The important thing

that we need to do in order to solve this problem is to use our communication skills, talk with her

slowly and lightly. Always listen to her and care about what she says. We try to understand and

empathize with her, show that we are caring about her story, and reduce her lonely feelings.

We were stuck when we tried to convince her about her eating. Although we explain that her

disease does not allow her to take glucose but she still wants to eat that. She told us that she is

lonely and she is going to die. So she did not need to plan and diet. But I think that my colleague

did well in her conversation with Mrs. Sherry. Due to her patience, she can convince her to not

eat chocolate until she has an allowance from her main care provider or doctor. In my opinion, if

she is given enough care from others including her family, nurses, etc, she will accept the

treatment and follow her nutrition diet. The experience that I learned from the case of Mrs.

Sherry treats her by the following five elements: power, trust, respect, professional intimacy,

empathy (CNO, 2006). Additionally, utilizing types of communication: verbal, non-verbal

communication such as body language. It is also effective when we need to convince the patient.

In this case, I can apply my knowledge as well as skills in order to practice. Personally, I suppose

that discussing together before talking with the patient is a good idea and it is essential to give a

good care plan for the patient. Discussing is very important whenever we work, I realize that my

colleagues have a lot of good ideas for the care plan of Mrs. Sherry that I can apply such as

discussing with her family members to help me improve her mood and have more motivation for

the treatment in the hospital.

Having played the role of a nursing student who has the task to care about Mrs. Sherry, I feel like

I am also an emotional supporter as well as a motivator in this whole experience I have gained

meaningful experience, which I can use in my future clinical practice.

I believe that every case that I approach is definitely different. However, based on the experience

I practice in class, I will be confident when I meet other cases as well as clients and I can apply

skills that I learned to those situations. The positive experience in this case of Mrs. Sherry is that

I can understand the importance of listening, patience, communication skills, and empathy with

the client. Practicing, in this case, helped me realize some things about the role of nurses. It is not

only taking care of the physical health of the patient but also paying attention to their mental

health. In order to achieve it, I have to utilize the experiences, knowledge I have learned.

Besides, I should discuss with my colleagues about care plans for the patient. It is teamwork

skills that are very necessary for me when I go to a clinical environment in the future.

By observing the situation during class time, I will apply some solutions, skills that I mentioned

before in the future when I work in a clinical environment. Additionally, I also will consider

ideas from my colleagues in order to provide good care plans for all clients that I will take care

of. So as to do my work well in the future, I need to practice more in class, read books or some

resources that can get me knowledge about nursing such as CNO website, etc. And after every

practice, I will look back, reflect on them. It helps me to have a deeper perspective, more

understanding of the problems of the patients as well as be more confident in the future when I

meet every situation.
