Sir:

This is to inform your good office that the PTA Officers and Members of the above named school
had expressed their intention through a resolution that the school will conduct a Summer Classes for
the students who failed in not more than two learning areas at the end of the school year 2017-2018
despite the educational interventions given.

In this regard, in behalf of the PTA officers, members and the school as whole, would like to
request your good office for an approval of our proposed SUMMER CLASSES starting April 15 (Monday)
until May 25, 2018 with a Final Examination on May 26, 2018 (Saturday) for a total of thirty five (35)
days.

Herewith is attached copy of PTA Resolution, school calendar for summer class, list of teachers
who intent to teach during the summer class, general class program for summer class, tentative list of
enrollees with learning areas to be taken and written consent of parents of students’ will attend the
summer classes.

Thank you very much for your usual favorable action. We are hoping for your approval and
granting us permission to conduct a Summer Class for the school year 2017-2018.
