1. Health & Safety Statement


Mohammed H. Hammam and Partners Company recognizes its responsibilities under the Health and Safety Law
of Kingdom of Saudi Arabia conjunction with Client’s Health & Safety Requirements, to ensure that its operations
are executed at all times in such a way as to ensure, as far as it is reasonably practicable, the health, safety and
welfare of all persons likely to be affected, including employees, sub-contractors, visitors to site and members of
the public. Mohd Hammam is committed to continual improvement of the health and safety management system

2. Health & Safety Policy & Procedure


In line with these responsibilities, Mohammed H. Hammam and Partners Company aims to provide, as far as is
reasonably practicable:

o A safe and healthy working environment for all its employees, the carrying out of risk analyses and the
identification of hazards.
o Systems of work, which are safe, and without risks to health.
o General training in health and safety so that all employees are made aware of any potential hazards in
relation to their work, and the latest procedures for overcoming them.
o Specific training in safe working procedures, the operation of plant and machinery and accident
prevention.
o A system for conducting a regular review of the training needs of the workforce.
o The provision of regular refresher courses, with reference to training already carried out.
o Basic training in health and safety to new employees and young workers, and to ensure that whenever
new plant or variations in systems of work are introduced, then the appropriate safety training is given.
o Personal protective clothing and equipment appropriate to the job in hand.
o Guidance on what to do in the event of an emergency.
o On-site welfare and first-aid provision, which complies fully with the statutory requirements together with
a system for maintaining the equipment.
o Adequate firefighting equipment and fire training, in liaison with the fire authority.
o A means of contact with the relevant health and safety bodies so that up-to-date information on
legislation, codes of practice and other technical guidance is available within the company, and is
circulated promptly to all concerned.

3. Safety Training Program


3.1 Training Program Descriptions

First Aid Training


The principle of this course is to help participants identify and eliminate potentially hazardous conditions in their
environment, recognize emergencies, and make appropriate decisions for first aid care. It teaches skills that
participants need to know in order to provide immediate care for a suddenly ill or injured person until more
advanced medical care arrives to take over.

Fire Safety training


The Fire Safety Training Course is designed to raise awareness of fire hazards in the workplace and how to act
safely in the event of an emergency fire situation, including selecting and using the correct fire extinguisher. The
course is also designed to train staff in your fire safety procedures.

Hazard Recognition training


The Hazard Recognition & Control Training course teaches participants the fundamentals of how to identify
hazards in the workplace, rate the risk of the hazards identified, and recommend controls to eliminate or minimize
the risks of these hazards using various tools developed by safety professionals.

H2S safety training


The aims and objectives of the Basic H2S Training are to ensure that the delegate gains the required knowledge
and understanding of the particular hazards and properties of H2S, and appropriate emergency response actions
to take should an H2S related incident arise. The H2S training program is for personnel that are or could be,
working in an environment that could become contaminated by H2S gas.

Confined space safety training


The course will educate participants on the definition of a confined space and the minimum site safety
requirements and procedures to be implemented when opening, entering, exiting, and/or performing work within
these spaces.

Permit to Work Training


This course is a requirement for all permit issuers/receivers working in ARAMCO/ SABIC sites.

4. Safety Program and Service Methods


4.1 Safety Policy & Assignment of Responsibilities
The Company's safety and accident prevention policies must be clearly understood by all employees, supervisors,
and management and must be stated in writing by the Company's management. All employees should
satisfactorily discharge the responsibilities of their job, and be aware that their safety record will be taken into
account during performance appraisals. The main responsibilities of various members of team are cited in the
following.
MHCO Senior Management
o Initiate the company's policy for the control of injury, damage and fire.
o Administer the policy himself or appoint a senior member of staff to do so.
o Ensure that all supervisors are qualified and that they receive adequate and appropriate training.
o Make sure that in tendering, at planning stages and throughout the contract, allowance is made for
suitable and sufficient equipment to enable the jobs to be done with minimum risk.
o Coordinate safety activities between SABIC and any other sub-contractors who may be working on the
same site.
o Institute a proper system for investigation, reporting and estimating the cost of injury, property damage
and fire loss. Initiate analysis to discover accident trends and promote action to prevent recurrence.
o Reprimand any supervisor for failing to discharge satisfactorily the responsibility allocated to him.
o Ensure that a formal Hazard Identification Plan is prepared in order to identify and correct hazards.

o Attend job progress meetings and submit a report on job safety performance or any Safety Violation write
up by PMT or Loss Prevention.
o Inspect the work site daily to report and correct unsafe methods and conditions.
o Keep a permanent record of all injuries, fires, motor vehicle accidents (MVA's) property damage and crane
/ heavy equipment accidents etc., which have occurred at the site.
o Keep a record of every weekly safety meeting on site complete with subject discussed and a list of
attendees.
Project General Manager
o Contract Project General Manager has overall and ultimate responsibility for all matters relating to
Health, Safety and Environment.
o He shall ensure that safety management plan & annual safety action plan are in place and are monitored
and reviewed for effectiveness.
o He shall seek clarification from SABIC PSM Coordinator for any requirement.
Project Supervisor:
o Understand the Company's safety Policy and the responsibility allocated to each grade of supervision.
o Ensure that tenders adequately allow for sound working methods and reasonable welfare facilities.
o He is responsible for cascading and enforcing SABIC Safety Policy / procedures requirements.
o Determine the following at the Planning Stage:
o The most appropriate order and method of performing the job.
o Allocation of responsibilities for SABIC, MHCO, and other sub-contractors.
o Storage areas, access, etc.
o Any hazards identified under the hazard identification procedure described herein.
o Facilities for welfare, first aid and sanitation.
o Work Permit procedures and requirements.
o Basic precautions for dealing with fire.
o Position equipment effectively and ensure that electricity supply is installed, used and maintained
correctly.
o Check that equipment and tools (both power and hand tools) are maintained in good operating condition.
o Make sure that all men know how to obtain and administer first aid properly and efficiently to all injured
persons. They should also know how to summon assistance in case of emergency and nominate others to
act in your absence.
o Make sure that suitable personal protective equipment is available and that it is used.
o Release supervisors and men when necessary for safety and fire training.
o Cooperate with the safety engineer and the fire department, by acting on their recommendations.
Project HSE Officer:
o Be familiar with those parts SABIC/Aramco Manpower Contractors Safety Manual applicable to the work
on which subordinate workers are engaged.
o Incorporate safety procedures in routine tasks and see that they are obeyed.
o Conduct weekly safety meetings with subordinates.
o Conduct daily work site inspections to identify and correct any existing unsafe conditions. Document and
coordinate the safety inspection activities and findings with the job site safety supervisor.
o Correct unsafe acts, such as horseplay or the taking of unnecessary risks.

o Ensure that new employees are properly instructed in precautions to be taken before they are allowed to
start to work.
o Commend men who, by action or initiative, eliminate hazards.
o Report accidents, unsafe conditions and defects in equipment to immediate superiors.
Field Supervisors
o Field Supervisors are responsible for implementation of safety Management Plan in all areas under their
direct control.
o Prepare reports include inspections, tool-box meeting and near miss reports.
o Enforce Safe Work Practices
o Correct Unsafe Conditions
o Prevent Lingering Unsafe or Unhealthful Workplace Conditions or Hazards
o Investigate Workplace Accidents
o Promote Quick Return to Work
Worker
o Use the correct tools and equipment for the job. Use protective clothing and equipment provided.
o Do nothing to endanger self or work mates.
o Keep tools in good condition.
o Refrain from horseplay and abuse of safety devices, equipment and welfare facilities.
o Report any accidents, near misses or hazardous conditions to immediate supervisor.
o Read the Company Safety Rules and take note of special safety precautions in restricted areas.
o Obey all posted warning signs.

4.2 Procedures
Risk Assessment
o Identify hazards, i.e. anything that may cause harm.
o Make a record of the findings
o If jobs are identified as high risk, a written risk assessment must be prepared in specified format.
o The Team supervisor’s circulate written risk assessment to their assistants.

Permit To Work (PTW)


Work shall be carried out in compliance with the SABIC/ARAMCO PTW procedure.

Emergency
On hearing the emergency alarm in the area of work, the following steps must be carried out by all
personnel:
o Stop work and turn off the equipment
o Assemble in the evacuation Assembly Area.
o If you are driving a vehicle, stop vehicle engine, vacate the vehicle and walk towards the Assembly
Area.
o Don’t return to work until notified from Emergency supervisor.

Welding / Cutting
As per SABIC/Aramco safety guidelines, the following steps to be completed before commencement of
work:
o Work area is cleaned from all flammable material.
o Running water supply is kept in the area right below the work location and keeps the area wet.
o Fire extinguisher is available at the work site and in good condition.
o Personnel executing the work are wearing PPE according to job requirement.
o Welding equipment is properly earthed.
o Welding machines must be inspected and in good working condition.
o Gas cylinders are equipped with flash back arrestor between regulator and hoses and check valve
between torch & hose.
o Inspect all hoses and regulators for cleanliness and working condition.
o Ensure that regulators are used properly and gauges are checked and calibrated.
o Make sure that hoses and fittings are inspected and are in good condition.

Confined Space Entry


o No one is allowed to enter a confined space, without proper permit to work order .
o Only appointed employees are allowed to enter confined space.
o Wear proper PPE and make sure all PPE’s are in good working condition.
o Train entry attendant and the watchman, their responsibilities in confined space.
o If work is stopped a “NO ENTRY” sign board must be displayed at the entrance of the confined
space.
Lifting operation/Equipment

o For newly installed, overhauled or changed minor cranes, the lifting test must be carried out in
accordance with the relevant provisions of the performance test of minor cranes before use.
o Before each shift of small crane operation, it should first do no-load lifting, rotation, amplitude
change, around and around operation and safety performance test of brake and limit device. If
equipment fails, it should be removed before formal operation.
o The driver of the small crane and the signalman shall communicate with each other according to
the prescribed gestures or signals. During the operation, the driver should cooperate closely with
the signalman and obey the command of the signalman. However, in case of danger in lifting
operation, the driver should stop immediately, no matter who sends the emergency stop signal.
o After the driver gets the lifting signal from the signaller, he must first ring the signal before lifting.
When lifting, the heavy objects should be lifted off the ground first. After confirming that the
heavy objects are fastened, the braking performance is good and the small crane is stable, the
lifting should be continued.

o When lifting heavy objects, the hook wire rope should be kept vertical. It is forbidden for the
hook wire rope to drag the lifted heavy objects in an inclined state. When the hook has been
hooked but the lifted weight has not been lifted, it is forbidden for the small crane to move or
rotate. It is forbidden to hoist and pull objects buried underground or condensed underground
or of unknown weight.
o When lifting and rotating heavy objects, the speed should be uniform and stable, so as to avoid
the danger of swinging heavy objects in the air. When laying down heavy objects, don’t go too
fast to prevent them from falling suddenly and being damaged. When hanging long or heavy
objects, special person should pull the slip rope. To prevent accidents caused by swing of heavy
objects.
o It is strictly forbidden for small cranes to work beyond the rated lifting weight of the machine. If
two small cranes are used to lift a heavy object at the same time, they must be under the unified
command of a special person. The lifting speed of the two cranes should be kept equal, and the
weight of the weight of the two cranes should not exceed 75% of the total rated lifting weight of
the two cranes. Attention should be paid to the distribution of weight and the weight shared by
each crane should not exceed 80% of the rated lifting weight.
o When lifting heavy objects by a small crane, it is not allowed to pass over the head of a person or
stay in the air for a long time. Under special circumstances, if temporary stopover is required, a
signal should be sent to inform all personnel not to stand or pass under the heavy objects.

Prevention of falling objects


o Use tool lanyards to prevent tools from falling.
o Keep all material at least 3 feet from a leading edge, other than material specifically required for
work in process.
o Remove items from all loose or unsealed pockets, especially top shirt pockets, such as phones,
pens, and tools.
o Do not hang objects over guardrails.
o Secure all objects when working on an elevated surface.
o Ensure toe boards are in place and inspected frequently
o Require hard hats and other required personal protective equipment (PPE) for every person in
areas at risk for falling objects—no exceptions.
o Rope off the area, if possible, where fall or drop hazards may exist.
o Inspect all PPE prior to use to confirm it still meets manufacturers’ recommendations.
o Communicate often. Work as a team to avoid complacency and remain vigilant of these
procedures at all times.
