May I present to you PRINCIPLE 3 ACCOUNTABILITY AND
CONTINUOUS IMPROVEMENT of School Based Management
System.
SLIDE 1 A clear, transparent, inclusive and responsive
accountability system is in place, collaboratively developed by community
stakeholders, which monitors expected and actual performance, continually
addresses the gaps and ensures a venue for feedback and redress.

Here is our School Monitoring Evaluation and Adjustment Team this
team composition is lifted from the School M & E Handbook of 2011.

We have accomplished SMEA Minutes of the meeting for first semester
and second semester and pictorials of our virtual meeting thru google

meet platform, attended by the Barangay officials, teachers, GPTA
officers, parents and the learners.
Then we have the Activity Completion Report or ACR for first and

attached here with the DepEd order number 54 series 2009 about the
revised guidelines governing parents-teachers associations PTAs at the
school level.

Now we head to our Enhance School Process which is our best practice
in our school on distribution and retrieval of modules. The project

The signed Memorandum of Agreement where in the function and
responsibilities of what to expect from the Barangay officials, SK
Officials, Teachers, Parents and Learners. Each Kagawad per purok
identified Community Learning center or CLC that will serve as drop off
point of modules. The parent living in the CLC will function as the PLF
or the Purok Learning Facilitator where in he/she acts as the seer if there
are still modules that are not yet pick up by the parents and tasked to

report it to the teacher adviser so that the teacher can call up the attention
of the parent. There are also Parent Leader per class that will pick up the
and distribute modules and collect and pass the answered module to the
CLC.

While the SK officials will act as the Big Brothers and Sisters that will
help the pupils with a difficulty in learning the lesson.
Here is the attendance for the stakeholder present on the crafting of

SLIDE 32 Project Guiteb Magkaisa para sa Edukasyon ng mga bata. Also the
identified CLC and PLF per purok and pictorials.

SLIDE 33 Let’s move now to Indicator 3 Feedback Mechanism.


SLIDE 34 In this indicator used the LESF as mode of feedback mechanism thru this
we are able to identify the Learning Delivery Mode this school year
which is Modular distance learning as some parent answered that they
don’t have devices to use for online classes. The enrolment this School
year went smoothly because the action taken by the Barangay Officials
headed by Kagawad of Education Hon. Panfelino Mauricio as they
identified strategic loction for enrolment drop off points these are the
Barangay hall and school where in LESF form are available. We also
posted Tarpaulin to these drop off point so that the parents will be
informed about the ongoing enrolment. We also implemented online
enrolment for the parents who are afraid to go outside their residence and
text to enroll form. The barangay also sent barangay tanod to go around
the barangay informing the parents to enroll their children this school
year. Even the Juan Guiteb a non-government Facebook page also join in
on endorsing the enrolment process.

Simulation videos were also posted on the School Facebook Account to


inform the parents on what to expect this School year.
We also reinforced the outsourcing of funds to meet the needs on
printing the modules every quarter. where in we posted a informative
flyer in the school’s facebook account to ask for their generosity and the
support is very evident from the stakeholders as the school outsourced

SLIDE 35 donation amounting to 80, 000 pesos.

As a way of thanking them we posted appreciation video on their


generosity on our school’s Facebook account. Attached here are the list
of donors and pictorials.
Now let’s have indicator 4 Stakeholder’s Recognition for our best

practices the Quarterly recognition of learners, parents, and stakeholders.
Here are the attachments for Learners recognition. List of awardees,

SLIDE 37 sample certificate and pictorials and the awarding was done via group
chat.
Next are the attachments for teachers’ awards the criteria on identifying

SLIDE 38 the outstanding teacher, School Learning Action cell we have attended
and the certificated awarded to the teachers.

Next is the awards received by the pupil and school

SLIDE 40 The school bags first place in Tiktok challenge in DRRM district contest.
Also, our pupil bags second place in poster making in the same contest
district level.
