Task Description
Work Description Hazards Identified Preventive Measures

Crane Mobilization Traffic Accident  Secure all necessary work permits prior to execute the job.
 Fix the traveling route and make sure the ground resistance satisfies traveling
requirement of crane.
 Flag man should be assigned to direct and guard when traveling.
 All employees involved in the operation to be aware of hazard
 Foreign materials within lifting area should be clear
 Crane should be positioned on hard and smooth ground to settlement
Crushed body parts,  Barricaded the swing radius
Crane positioning
cut fingers/hands  Flag man required
 Use proper PPE
 Tool box meeting to be conducted by supervisor before start job.

Loading/ Unloading Steel Structures. Crane Collapse  Crane operator must be physically fit and no mental illness.
 Crane shall be checked in order to ensure that there is no leak of hydraulic oil
or any other defects, before starting job
 Crane must have valid 3rd party test certificates.
 Crane operator shall have Saudi equipment operator license in addition to third
party.
 Crane load chart to be checked.
Hitting & Bump of the  Capacity and position of the crane has to be checked.
Materials  Crane out riggers fully extended & pad to be conforms as per DIC standard.
 Authorized signalman to be give signal to crane operator.
 Barricaded the swing radius.
 No erection allowed during harsh weather e.g. wind exceeding 20mph, thick
fog, heavy sandstorm, rain etc.
 Wear proper PPE's for the job (hand gloves)
 Certified riggers do the preparation / fastening of slings & belts
Preparation of steel members to be
 The capacities of the slings & shackles have been checked/approved for the
erected Pinch point load.
 Softeners & taglines are provided for the load.
 Certified riggers designated “signalman-in-charge" for the lift
 Riggers are detailed in upper & ground level. As required.
Failure of sling/cut  The riggers give consistent signal to the crane operator
Lifting/positioning & erection of steel Breakage of D-Shackle  The crane lift radius area is clear from any workmen
structures on specified location personnel injury  Lifting activities is coordinated with other interfacing subcontractors.
/elevation cut  Proper communication between riggers & crane operator ensures.
Fall hazards  Ensures that steel structure is cleared of loose object prior lifting
Human fall  100 % PPE compliance - full body harness w/ lanyards.

 rigger & steel fixers are restricted to step outside or on top of railings
 steel fixers are restricted to step outside man lift when @ height

Bolting of steel structures Slip & trip
 ensures safe access
 Scaffold access & flat forms are tagged safe (green) for use.
 All hand tools should be secured with by a rope tied to the harness.
Fall of object
 Barricade the fall zones and provide signage
 Safety helmet with chin straps, safety glass.
Man lift failure
 Certified man lift operator designated,
 Trashes, use hand gloves, plastic, wires must be collected and properly
Housekeeping prior knock-off time disposed -off into the trash bin.
