North Coast Semmes-Weinstein Monofilaments
Indications
The North Coast Semmes\Weinstein Morioflaments arc used 19
evaluate cutaneous sensation levels thréughout the body. Using
‘monofilaments is indicated in diagnoses including nerve com:
‘Pression syndromes, peripheral neuropathy, thermal injuries,
and postoperative nerve repair.

radial nerve (See Figure 1),
6. Grasp the monofilament handle and flip open the monofilament so
that it clicks in place ata 90° angle to the handle (See Figure 2),
When fipping the monofilament into the testing position, do noe
cover the channel with your finger. Covering the channel may
‘cause the filament to bend as it ips into the testing position.

7. Press the monofilament against the skin until it bows. Hold the
‘monofilament in place for 1.5 seconds, remove for 1.5 seconds
and repeat. For monofilaments from 1.65 to 4.08 apply the stim-
lulus up to three times to elicit a response. For filaments 4.17
through 6.65, apply the stimulus one time only.
8. Begin with the 2.85 flament. Ifthe patient is able to detect the
2.83 filament, the exam is complete. If the patient does not
Fespond to the stimulus, choose the next largest monofilament
and repeat the procedure. To maintain the patient's attention
during testing, intermittently move the monofilament to test
areas known to have sensation,

Instructions for Care
1. Clean the monofilament handle with a damp sponge or cloth
‘using mild soap. Monofilaments can be disinfected using a cold
soak in an instrument disenfectant.
2. When not in use, protect the monoflanent by otatng i into the
closed position so that the filament rests into the channel in the
handle. A bent monofilamerit may give false results,
5. When placing or removing the monofilament handle into or from
the carrying case, grasp the handle at the NCM logo to prevent
rotating the filament against the case.
9. Record results using colored pens that correspond to the colors
on the filament handle, as noted a the chart below. You also
‘may choose to record the results using icons that note areas of
normal, delayed, or absent response. (The use of icons will fact.
itate copying or faxing the recording form). Responses delayed
‘more than three seconds should be recorded as “delayed” on
the form. To help screen out false positives, vary the testing, pat.
tem, including time intervals between different monofilaments.
