I, ALVI JEAN GAMUL CONDES, Filipino, of legal age and with residence
address at 585 Luksuhin Ibaba, Alfonso Cavite, after being duly sworn to in
accordance with law, hereby depose and state:

1. That on June 20, 2022, I resigned as Operations Supervisor from Convey


Health Solutions with address at 18th Floor, Cybergate Tower3, Pioneer St.,
Mandaluyong City;

2. That in relation to my resignation from the said company, I understand that it


is no longer the obligation of Convey Health Solutions to pay my salary loan
with the Pag-IBIG and SSS;

3. That I will continue to pay said loans until it is fully paid;

4. That I am executing this affidavit for all legal intents and purposes it may serve
and to attest to the truth and veracity of all the above mentioned statements.
