Teacher II

 Bachelor’s Degree for Teachers


 3 or more teaching experience plus 20 units in M.A. or
 9 years teaching experience plus 18 units in M.A. or
 20 years teaching experience without M. A. units

Teacher III

 Bachelor’s Degree for Teachers


 3 or more teaching experience plus completed academic requirements (CAR) in Master
of Arts or M.A. graduate or
 20 years teaching experience plus 20 or more units in M. A.

Requirements (Please file according to the sequence below)

1. Endorsement letter from the school principal


(2 original copies)
2. Duly Accomplished ERF (5 copies)
3. Certification from the school registrar for M.A units
Earned/CAR (1 original copy and 1 photocopy
4. Transcript of Records (1 TOR Authenticated and
1 photo)
5. Updated Service Record (3 original copies)
6. Certificate of Performance Rating for the last 3
School years. (Signed by administrative officer/SDS/
School principal- 2 copies). Also attached the
Photocopy of IPCRF and PAST
7. Sworn Statement(Omnibus) that all documents
submitted are true and correct with documentary
stamp( 2 original and 1 photocopy)
8. Position Description Form (3 original)
9. Duly accomplished Form 212(Personal Data Sheet,
1 original copy
10. Latest Appointment (1 certified copy)
11. Latest Pay slip or Payroll ( 1 certified true copy)
12. Certificates of trainings, seminars, workshops and
Awards,3 of 3. (certified true copy)
