III. PROBLEM PRESENTED:

The client’s is a Person with Disability (PWD) member of the association of Barangay Poblacion, Buug Zamboanga
Sibugay. The client is verified to a polio survivor/orthopedic disability and belongs to a poor family and desires to avail of
livelihood assistance from the Provincial Social Welfare and Development Office Ipil, Zamboanga Sibugay.

IV. BACKGROUND INFORMATION:

Client is the sole breadwinner of the family while her wife is a plain housewife taking care of her 9 children. Client is
suffering from a stroke last 2017 and his condition has had a significant impact on his daily life. The client works as a security
guard at Msu Buug Campus and receives a minimal amount, only enough to provide for the essential needs of the family.

Client managed a small sari-sari store as one of their sources of income. The family is dependent only on the daily
income of their sari-sari store since client income is insufficient for family needs. The family is hoping that they can avail
livelihood assistance, though he expressed that he has a skill in business management with an assurance that he can send his
children to formal education. Hence, the client is humbly asked to avail of the assistance for the procurement of the groceries
for their small sari-sari store.

V. RECOMMENDATION:

Client and family are indigents and in dire need of financial help. To be able to augment her financial needs for his
capital and purchase of grocery goods, Undersigned recommends Livelihood Assistance under the Program from Provincial
Social Welfare and Development Office, Ipil Zamboanga Sibugay in the amount of FIVE THOUSAND PESOS (5,000.00)
