CEREBELLAR EXAMINATION
Hi I am divya one of the medical students at windsor
university school of medicine .
- Can i check with your name and date of birth
please - Okay great! I would like to test your coordination today , so it’s gonna involve me observing you walking, moving arms and also testing your eyes. - Is that okay? - Can we start ? Okay can you please walk towards ( assessing gait,stability )
- I will ask the patient to walk with one foot in front
of the other back to the other wall ( tandem gait- it exaggerates unsteadiness) - I am gonna test your balance now , so if you can put your arms down by your side and feet together, i’m just gonna put my arms 0ut if you feel unsteady …. So can you just close your eyes for me now…( ROMBERG’S TEST positive if the patient falls without correction , this is a test of proprioception rather than cerebellar disease) Now you can open your eyes.
- INSPECT PATIENT’S POSTURE- note any
truncal ataxia - ASSESS SPEECH - slurred staccato speech is characteristic of cerebellar disease. ‘I am just gonna ask you to repeat a few words for me to test your speech” Contemporary ; spectacular; - I m just going to ask you to follow my finger with your eyes try and keep your head still as you can ASSESS FOR NYSTAGMUS - ( If nystagmus is present note direction of gaze and nystagmus) - ASSES COORDINATION finger to nose test -past pointing is noted in cerebellar disease. So now if you can touch my index finger with your right hand and touch your nose and touch my finger and keep going back and forth . Can you repeat it with the other finger . Assess for rebound phenomenon- now if you can put your arms out straight for me like this and if you can close your eyes now i’m going to push down on your arms one either side just try and keep your arms ups for me.
ASSESS TONE - SO i m just gonna move your
arms around so just let me take your weight and just go floppy , have you got any pain in your arm at all? On both sides. (HYPOTONIA IS NOTED IN CEREBELLAR DISEASE.)
DYSDIADOCHOKINESIA - inability to perform
rapidly alternating movements. - Can you place one of your hands on top of the other like this for me and just turn the top over and keep doing back and forth again as quick as you can , - Now with the other hand.
Now I’m gonna test your reflexes now so i'm
gonna be tapping your knees on both sides. Knee reflex - hyporeflexia in cerebellar disease. Okay so now if you can swing your legs around in the bed Now if you can place this heel on that knee for me and then just run it down the front your leg and lift it up and go back to the knee and please keep doing it. Incoordination is noted in cerebellar disease ASSESS COORDINATION - HEEL TO SHIN TEST. Can you repeat it with the other leg.

Today i performed a cerebellar examination on --- yr
old . On assessment of the gait there was no evidence of a broad based ataxic gait , there was no loss of coordination of trunk or limbs no nystagmus in eyes . it was a normal examination
