AFFIDAVIT OF DISCREPANCY

I, LOLITA C. GILO, of legal age, Filipino and a resident of City of Ilagan, Isabela, after
having been sworn in accordance with law, hereby depose and state:

1. That I am the same person executing this affidavit;

2. As per CERTIFICATE OF LIVE BIRTH, duly by the Philippine Statistics Authority (PSA),
my registered birth date is “SEPTEMBER 18, 1970”;

3. However, upon looking in my records from the Commission on Elections (COMELEC),


my birth date is erroneously registered as “OCTOBER 12, 1970” instead of
“SEPTEMBER 18, 1970”;

4. That this instrument is being executed with the purpose of clarifying my date of
birth with no intention to defraud anyone;

5. That the person with the birth date “SEPTEMBER 18, 1970” which appeared in the
CERTIFICATE OF LIVE BIRTH and the person named as “OCTOBER 12, 1970” which
appeared in the COMELEC records, refers to the one and the same person;

6. That I am executing this instrument to attest to the truthfulness of the foregoing


facts and for whatever lawful purpose it may serve.
