CLSU CAT 2023 Application Procedure

Visit the CLSU College Admission Test portal and create an account with your email and password. You will use this to log in to fill out the online application form with personal details and educational background. Upload PDF copies of requirements like birth certificate and Form 137-A and 138. PLEASE READ AND UNDERSTAND ALL THE STEPS AND MAKE SURE ALL
DOCUMENTARY REQUIREMENTS ARE READILY AVAILABLE BEFORE YOU
PROCEED WITH THE ONLINE APPLICATION.

STEP 1
CREATE AN ACCOUNT
A Fill in your Last Name, First Name and Middle Name.
Provide your active personal email and request for an OTP. Supply

the 6-digit One Time Password sent to your email.
C Indicate your date of birth.
Create and confirm your password (at least 8 characters). Remember

this password because you will use this every time you sign in to the
portal.
E Click submit to complete the process.

Do not create multiple accounts. Your application may be invalidated.


Once your account is successfully created, you can now SIGN IN using
your email address and password.

STEP 2
FILL OUT THE ONLINE APPLICATION FORM
Make sure to supply all required information. There are 4 parts to be
completed which include:
A Category of Applicant

B Course Choice
C Personal Information*
D Educational Background

*NOTE: In this part, you will be asked to upload an electronic copy of your
recent picture (2x2 colored, white background, with nametag: SURNAME,
FIRST NAME, MIDDLE INITIAL, formal pose, no eyeglasses and accessories,
file size less than 2MB)

Make sure to review all information you provided before you click the
submit button. Once submitted, you cannot edit the application form.
STEP 3
UPLOAD THE NECESSARY DOCUMENTARY
REQUIREMENTS, SAVED AS PDF FILES
These include:
A Scanned copy of PSA/NSO birth certificate;

Scanned copy of Form 137-A (Junior High School, Grade 7-10),

reflecting the grade point average (GPA) in each grade level, and
duly signed by the school principal or school registrar*;

Scanned copy of Form 138 (Grade 11), reflecting the grade point
C
average (GPA) in each semester, and duly signed by the school
principal or school registrar*
* Note: For Alternative Learning System (ALS) graduates- Certificate
of Rating/Equivalency issued by the Bureau of Education
Assessment.

It is recommended that you name the file appropriately to avoid


incorrect/improper uploading.

STEP 4
DOWNLOAD AND PRINT (8.5x13) THE
APPLICATION FORM
After printing, affix your signature above your name. Ask your parent/
guardian to do the same. Do not forget to indicate the date. You will bring
this signed application form, as one of the requirements during the day of
the examination.

DOWNLOAD AND PRINT (8.5x13) THE
TEST PERMIT
The test permit includes information on the venue, day, and time. It also
includes important reminders, so read it carefully. You will bring the test
permit, signed application form, and photocopies of all documentary
requirements during the examination day.

FEBRUARY 11-12 & 25-26, 2023
