Gordons Functional Health Pattern

The patient has a history of type 2 diabetes mellitus, hypertension, and cataracts with retinopathy. He now has end-stage renal disease stage V, which affects his mobility and normal activities as he needs assistance to walk. His illnesses are believed to be caused by his diabetes. He now controls his food intake as he previously ate a lot of sweets and soda. He has difficulty caring for himself due to vision loss. On examination, his blood pressure was elevated and he had edema, darkened skin areas, and a foot ulcer. He eats 3 times a day and previously ate a lot of sweets. He has normal bowel and urinary patterns. He is limited in physical activity due to his condition and vision

Gordons Functional Health Pattern Health Perception- Health Management Pattern Subjective Data: His past illness was

DM type II, HTN stage II and cataract retinopathy. For his present illness he has ESRD stage V. Because of his present illness his normal activities was affected. He cannot walk without holding other peoples back. He believed that the main cause of his illnesses was diabetes mellitus. He now control his intake of food because before he was into sweet foods most of all sodas. He has the difficulty in taking of himself because he already lost his eyesight. Objective Data: VS-BP 160/100, RR 15, PR 70, T 35.5 C

Nutritional Metabolic Pattern Subjective Data: His diet habits is 3x a day. He always eat sweet foods most of all sodas. He completes his meal from breakfast until evening meal. He didnt took any vitamin supplements. The condition of his skin was dry. Objective Data: Edema on the left arm, dark pigmented areas on extremities, foot ulcer on the left 5th digit

Elimination Pattern Subjective Data: He has normal bowel pattern. He has his bowel movement once a day. The appearance of the stool was normal. He dont feel any discomfort with his bowel movement. He frequently urinate about 3-5x a day. The amount and the color of urine was normal. He dont feel any pain while he was urinating.

Activity Exercise Pattern Subjective Data: His physical activities on the normal day was walking. But now because of his present illness he has the difficulty in walking. He was always help by his mother to walk. He always hold the back of his mother for him to walk and also one of the main factor why he needs help because he already lost his eyesight Sexuality-Reproductive Pattern Subjective data: He verbalized that he was already satisfy of what he has Objective data: Normal genitalia

Sleep-Rest Pattern

Subjective Data: He has a regular sleeping habits.
