PROCESS RECORDING FORMAT (use of therapeutic communication)

Description of the client: The patient was anxious, depressed and worried. She avoids eye contact and was very much restless and disturbed.
Description of the Environment The environment of the room was calm, silent, and has a room temperature. The room was locked from the inside to avoid noise and to
insure the privacy of the patient.
