AFFIDAVIT

         I, ARIEL A. DAGSIL, of legal age, Filipino, married and a resident of Motong,
Dumaguete City Negros Oriental, after having been duly sworn to in accordance with law,
depose and state:

1. That I am a Senior Credit Investigator/Collector of PNJ INT’L LENDING CORP.


Dumaguete;

2. That I am authorized to file a Small Claims Cases in behalf of the Corporation by


virtue of a Board Resolution No. 22-004 Series of 2022;

3. That in virtue thereof, I am filing a small claims for the collection of PASTDUE
ACCOUNTS for the amount of PHP34,003.75 against JUN RUSKIN A.
MONOFOLLO, of legal age, Filipino and a resident of 4576 Buñao, Dumaguete
City;

4. That JUN RUSKIN A. MONOFOLLO borrowed/loaned the amount of THIRTY


THOUSAND PHILIPPINE CURRENCY (PHP30,000.00) from PNJ INT’L
LENDING CORP evidenced by a promissory note duly notarized by Atty. Ronel
Depalubos, entered as Doc No. 371, Page No. 76, Book III, Series of 2021. Attached
hereof is the copy of the Promissory Note;

5. That JUN RUSKIN A. MONOFOLLO failed to pay said loan and despite repeated
demands, he refuse to settle his obligation. Attached is the copy of the demand letters
that was duly served against him;

6. That I am executing this Affidavit to attest the truth and veracity of the foregoing and
for the purpose of filing the small claims complaint against JUN RUSKIN A.
MONOFOLLO.
