BLOOD PRESSURE RECORDING

PREPARATION

PATIENT.

1. The patient should be resting in a bed, couch or chair , in a quit location with the leg uncrossed.
2. The patient should not have a meal, alcohol or caffeine or have smoked or exercised in the
previous 30 minutes.

EQUIPMENT.

1. Spigmomanometer with appropriate size cuff.


2. Stethoscope.
3. Alcohol impregnated swab.
4. Observation chart.

NURSE.

1. The hands should be clean.


2. No special preparation is necessary unless required by the patient’s conditions.

PROCEDURE.

1. Asses the patient’s knowledge of the procedure and explain as necessary.


2. Ensure the patient is resting in a comfortable positions. If a comparison between lying and
standing blood pressure is required, the lying recording should be done first.
3. When applying the cuff, no clothing should be underneath it. If clothing constrict the arm,
remove the arm from the sleeve.
4. Apply the cuff such that the center of the bladder(bagian tengah manset) is over the brachial
artery 2-3 cm above the antecubital fossa.
5. The arm should be positioned so that the cuff is level with the hearth and may be more
comfortable resting on a pillow.
6. The spyhgmomanomater should be placed on a firm surface with the dial clearly visible.
7. Perform radial check to estimate systolic blood pressure.Locate the radial pulse.Squeeze the
bulb slowly to inflate the cuff while still feeling the pulse.Observe the dial and note the level
when the pulse can no longer be felt.Open the falve fully to quickly release the pressure in the
cuff.
8. If using a communal stethoscope, clean the earpieces with an alcohol impregnated swab.
Curving the ends of the stethoscope slightly and place the earpieces in your ears.Check that the
tube are not twisted.
9. Check that the stethoscope is turned to the diaphragm side by tapping it.
10. Palpate the brachial artery which is located on the medial aspect.
11. Place the diaphragm of the stethoscope over the artery and hold it in place with your thumb
while your fingers support the patient’s elbow.
12. Position your self so that the dial of the spyghmomanomater is clearly visible.
13. Ensure that the valve on the bulb is closed and inflate the cuff to 20-30 mmhg above the systolic
blood pressure (step 7).
14. Open the valve to allow the needle of the dial to drop slowly(2 mm per second )
15. While observing the needle of the dial as it falls, listen the korotkoff sound these are :
16. The systolic pressure is the level where these are first heard
17. The diastolic pressure is the level where the sounds disappear.
18. Once the sounds disappeared, open the valve fully to completely.
19. Replace equipment and clean the ear piece of the sthetoscope.
20. Chart the blood pressure accurately.
21. Report any variations from previous recordings.
