Please submit on or before 15 February 2022 together with this checklist the following Barangay VAW
Desk Functionality Assessment MOVs in A4 size bond paper:
Copy of Barangay Ordinance or Executive Order establishing the Barangay VAW Desk and designation
of Barangay VAW Desk Person;
Copy of Certificate/s of trainings/seminars and workshops attended on VAW;
Copy of Certificate/s of trainings/seminars and workshops attended on Gender Sensitivity;
Copy of Certificate/s of trainings/seminars and workshops attended on Basic Crisis Interventions;
Photos of the VAW Desk location in the barangay hall;
Photos of the VAW Desk separate room for intake interview in the barangay hall;
Photos of table and chair in the VAW Desk separate room for intake interview;
Photos of filling cabinet/storage area of VAW cases and related documents;
Photos of sofa bed, folding bed, or mat in the VAW Desk separate room for intake interview;
Photos of barangay vehicle to convey VAW victim-survivor;
Photos of barangay landline or mobile phone;
Photos of barangay computer or typewriter for logging in and monitoring of VAW Cases;
Photos of barangay camera used in documenting proof of physical violence;
Photos of barangay voice recorder in recording the case narrative;
Photos of barangay First aid kit and other medicines;
Photos of Logbook 1 for RA 9262 cases;
Photos of Logbook 2 for other VAW-related cases;
Copy of blank and filled-out VAW Docs Intake Form;

Copy of 2021 Annual GAD Accomplishment Report based on the approved GAD Plan and Budget;
Copy of 2021 VAW monthly or quarterly reports;
Photo of database/records of all VAW cases reported in the barangay;
Copy of the 2021 1st Semester State of Barangay Address (SOBA);
Copy of the 2021 2nd Semester State of Barangay Address (SOBA); and
Copy of post activity report/s of 2021 activities conducted to eliminate VAW in the community.
