INDICATORS IN MBALE REGIONAL REFERRAL HOSPITAL, EASTERN UGANDA

INTRODUCTION

BACKGROUND

Rational Drug Use refers to patients getting medicines appropriate to their individual clinical
conditions, in patient-tailored doses, for an adequate time and at lowest cost to them and their
community (WHO, 2005).

For Rational Drug Use to be ensured, the physician, dispenser and patient should play their part
appropriately. The physician comes up with the correct diagnosis and suggests safe, effective and
affordable medication for the benefit of the patient. Dispenser practices good stock management,
provides a right drug to the right patient in right formulation and dosage, proper counselling and
clear instructions. Then the patient complies and adheres to the provided medication (Mamo and
Alemu, 2020).

On the other hand, Irrational Drug Use is the use of medicine in a way that is not compliant with
Rational Drug Use. Examples of Irrational Drug Use include; polypharmacy, inappropriate
antimicrobial use, injections overuse, inappropriate self-medication with prescription-only
medicines and failure to prescribe in accordance with Clinical Guidelines (Sema et al. 2021).

Irrational Drug Use has been reported worldwide with the prevalence highest in Sub-Saharan and
low income countries with prevalence of 89% in Tanzania (Mboya et al., 2018), 51.4% in
Zambia (Lukali and Mchelo, 2015), and 23.3% in Egypt (Sabry et al., 2014).

This always results into rapid emergency of anti-microbial resistance, increased mortality and
morbidity, high drug therapy costs, increased occurrence of preventable adverse drug reactions,
drug interactions and poor patient adherence (Sisay, 2017).

To avoid these Irrational Drug Use consequences, a country and its healthy facilities must have
interventions such as; use of Clinical Guidelines, development and use of National Essential
Medicines List, establishment of Drug And Therapeutics Committees, continuing in-service
medical education, public education about medicines, avoidance of perverse financial incentives
and sufficient government expenditure to ensure availability of medicines and staff (WHO,
2021).

This study will evaluate the degree of rational drug use in MRRH and factors contributing to
that, then its findings will be used to propose facility-tailored interventions to promote rational
drug use.
