Q1) What might happen when each line of business, division and department develops

its own IT apps?


Ans: In general, any organization possess a lot of data regarding their different departments.
If there is any misplacement, it will cause severe damage to the entire system. Some
organizations have individual IT application for their individual departmental variations.
1.Data Chaos which will cause the errors in the data and produces
irregular data.
2.Faulty data, which include the manual report as well as practical data may give
the irrelevant data representation and make the organization into degrading
position.
3.Data breaches is the anther risk we are going to face when all departments have
different IT apps. This include stealing of official data from an organization
without any intimation.

Q2) What are the consequences of poorly managed apps?


Ans: Poorly managing apps may include data chaos, data silos, data breaches, fault data.
Whatever the risk, there is a lot of loss for any field.
For example: Take an example in the case of hospitals. They all maintained the data of
their different departments running with different apps. This may cause the result of
losing of patients data and get irrelevant data in some case. This lead to damage to the
patients. In some cases it will cause the fall of organization reputation. In 2009, U.S
department of health and Human Services report this issue especially on data breaches.
Totally, there are four main consequences for poorly managed apps. They are:
a) Data Chaos
b) Data breaches
c) Data silo
d) Faulty data

Q3) What two risks are posed by data chaos? Explain why.
Ans: Data Chaos will cause the data errors and irrelevant data. Assume the case of
hospitals. The two risks are given as
1. Faulty Errors:
In case of medical field, at the end of 2016 the number for the healthcare will
opt for 80% of the Electronic Health Records (EHRs) . But a study conducted
by the Columbia University stated it is false result and this is due to the
occurrence of irrelevant data and incorrect lab records cause the change in data
and give faulty details of patients which lead to the loss for people.

2. Data breaches:
By 2009, U.S Department of Health and Human Services, revealed that
millions of people have lost due to this data breaches. This is caused due to the
misleading the data through systems, extendable data drivers. “Data Breaches
are extremely expensive and destroy trust”.

Q4) What are the functions of data governance in the health care sector?
Ans: HIMSS (Healthcare Information and Management Systems Society) is an organization
which is social and motive is to help for the society. This included the seven stages for
EHR,where in Stage seven, all the health departments in a hospitals get benefited by the data
governance methods because of following advantages:
i) All the medical records in from different departments the data is
collected and stored electronically through HIE (Health Information Exchange).
By this technology, the data can be transformed easily and accessed by the
physicians, patients and other health related organizations in a completely secured
way.
ii) They are more vulnerable to hacking and avoid data breaches completely. This
will secure the patients information.
iii) “IT investments and tools were increasing fast, but these are not under the control
of HIM (Healthcare Information and Management) policies.

Q5) Why is it important to have executives involved in data governance projects?


Ans: Intially Vanderbilt University Medical Center is taken under for implementing
electronic methods of storing data in an organization. It gets failed because of some
problems in data governance. Hence it is taken up by the consultants. They form a team
for making organized structure for data governance and take the issue to the notice of
medical board and make them get involve for sponsorship.
The medical board form an executive group to provide more detail to EHR policies.
This team helps for explaining the process for correction, adding, removing of duplicate
records from the record for all the members including staff, legal counsel, nursing
department, and chief medical information officer (CMIO). This team reduced the
complications with data governance, minimized the risks.

Q6) List and explain the costs of data failure.


Ans: The list of cost of data failure are given below:
1. Rework: The data kept as it is and can be available to retrieve whenever they
required.
2. Loss of business: If the electronically data is accessed when it is under maintenance.
Then there may be chance for data loss.
3. Patient safety errors: If the staff get wrong information regarding a patient, there
may be chance of harming.
4. Malpractice lawsuits: An organization must be thrust worthy towards its customers.If it
fails in doing so, then it fall under malpractice. This will lead to down the
organization reputation and may legally banned sometimes.
5. Flexibility: Data should be flexible for both staff and patients. If it delays there is a
chance for vexing of patient.

Q7) Why are data the most valuable asset in health care?
Ans:Now-a-days, medical field is growing very fast. New technologies are arriving in
making the work easy. Each and every patient record should be maintained so that we
access data at anytime, anywhere. Data of patient may include reports, bills invoices,
medicine details, patient’s personal details, appointment details should be saved so that
there may be a chance to require them in future.This can be legal and the data can be
authorized in such a way that patient can access
data and verified if it is true or not. Handling of data manually is of total risk, that why
