AFFIDAVIT OF INSERTION

I, TRIVEN P. CASTILLO, married, of legal age, Filipino citizen, with resident at 2/F
Gozos Bldg., F. Manalo St. Lipa City, after having been duly sworn in accordance with law,
hereby depose and says;

1. That I have notarized a certain documents Deed of Absolute Sale dated Jan. 6,
2021 executed by Salvador L. Madrigalejos married to Olympia C. Madrigalejos
in favor of Southern Medical and Health Care Corporation and entered in my
Notarial Book as per Doc. No. 332. Page No. 68 Book No. XXXIX Series of 2021

2. That there was an omission in entry in the said documents particularly the
marital status of the husband and the name of the representative of the buyer.

3. That I have inserted the said entry and countersigned the same as follows;
a.) Salvador L. Madrigalejos ‘’widow, now” married to Olympia C.
Madrigalejos
b.) SOUTHERN MEDICAL AND HEALTH CARE CORPORATION represented by
Dr. John Gerard A. Villa and Dr. Pepito S. Tenorio Jr.

4. That I am executing this affidavit to attest to the truth of the foregoing


instrument and for whatever legal purpose it may serve.
