REQUIREMENTS TO THE SANGGUNIAN

FOR CIVIL SOCIETY ORGANIZATION (CSO)

√ 1. Duly accomplished Application Form for Accreditation;


√ 2 2. Letter of Application;
√ 3 3. Duly approved Board Resolution signifying intention for accreditation for
the purpose of representation in the local special body;
√ 4 4. Certified true copy of Articles of Incorporation or Articles of Cooperation
and By- Laws;
√ 13 5. Certified true copy of c or Certificate of Accreditation with either of the
following:
__√_Securities and Exchange Commission (SEC),
_____Department of Labor and Employment (DOLE)
_____Cooperative Development Authority (CDA)
_____ Certification issued by NCIP in the case of IPOs,

√ 14 6. List of current Officers and Members;


7. Original Sworn Statement, issued by the Chief Executive Officer or
equivalent officer of the CSO, stating that the CSO is an independent, non-
partisan organization and that it will retain its autonomy while pursuing the
advancement of the peoples’ interest through its membership in a local special
body, after satisfying all the requirements and set criteria, as stated in this
Memorandum Circular, and after securing a Certificate of Accreditation from
the concerned Sanggunian.
√ 22 8. For existing CSOs, Annual Accomplishment Report for the immediately
preceding year;
√ 22 9. Pictures and other documentations of projects, programs and activities
undertaken in the preceding year.
√ 49 10. Existing CSOs shall submit Financial Statements for the immediately
preceding year certified by an Accountant, if income is more than P50,000.00,
or certified by its Auditor when its income is P50,000 or less, and indicating
therein other information such as sources of funds.
√ 53 11. For existing CSOs, Minutes of the Annual Meetings of the immediately
preceding year as certified by the organizations’ board secretary;
12. The documents should be 2 copies and placed in a folder arranged
according to the sequence of the requirements as enumerated above.

The officers and members of APO Roxas Oriental Mindoro Alumni Association (APO- ROMAA)-
a civil society organization, have requested me to convey to you the following for your kind information
and favorable consideration:
1. THAT our organization is signifying intention to apply for accreditation for membership in a
local special body manifested in the herewith attached RESOLUTION NO. 2021-10-A,
Series of 2021.
2. THAT our organization has a clear institutional vision and mission furtherly supported and
strengthened by our organizational goals and objectives;
3. THAT we unequivocally declare our unswerving belief that once accredited, we could truly
serve as “ active partners in the pursuit of local autonomy”, which is the heart and soul, so
to speak, of Republic Act No. 7160 , otherwise known as the Local Government Code of
1991;
4. AND THAT we are optimistic that with this looming partnership at hand, we can move onward
to greater bounds in committed and pro-active community service.
FURTHERMORE, submitted herewith are the documents and papers as required for our
accreditation.

A RESOLUTION SIGNIFYING INTENTION TO ACCREDIT WITH THE LOCAL


GOVERNMENT OF ROXAS AND TO BE A MEMBER OF A LOCAL SPECIAL BODY
Whereas, the Local Government Code of 1991 provides for the accreditation of non-
government and peoples organizations;

Whereas, the Department of the Interior and Local Government, in a Memorandum, set the
guidelines for the Accreditation of NGOs and POs;

Whereas, our organization, ALPHA PHI OMEGA- ROXAS ORIENTAL MINDORO


ALUMNI ASSOCIATION (APO-ROMAA), is a non-government/people’s organization duly
registered with the Securities and Exchange Commission and existing under Philippines laws;

Whereas, the Board of Trustees/Directors, in a special meeting discussed our accreditation and
our desire to take part actively in the formulation and implementation of local policies via our
membership in local special bodies;

Now Therefore, be it RESOLVED as it is hereby RESOLVED to SIGNIFY OUR


INTENTION TO ACCREDIT WITH THE SANGGUNIANG BAYAN, PURSUANT TO THE
PROVISIONS OF SECTION 108 OF RA 7160, AND TO APPLY FOR MEMBERSHIP IN A
LOCAL SPECIAL BODY CREATED UNDER THE AFOREMENTIONED ACT;

RESOLVED, FURTHER, to forward a copy of this RESOLUTION to the Chair of the


Sanggunian Committee on Accreditation, Honorable Jigs M. Talens.
