GENERAL REQUIREMENTS:
1. Duly accomplished and signed application form (form must be computerized, the signature must be
handwritten, font style and font size is Arial 12 only)
2. Two passport sized colored picture (white background)
3. Original passport (at least 6 months valid beyond intended stay)
4. Copy of passport data page
5. Original copy of valid visas and arrival stamps to OECD member countries for the past 5 years
6. Original personal bank certificate must include account type, current balance, account opening
date, 6 months average daily balance (ADB) (SAVINGS OR CURRENT ACCOUNT ONLY)
7. Original bank statement or certified true copy of passbook for the last 3 months (must be the same
bank account with the bank certificate)
8. Photocopy of ITR (Income Tax Return) of Form 2316

Required Additional Documents:

EMPLOYEES:
 Original employment certificate (must include applicant’s position, date hired, compensation, office
address HR landline numbers, and e-mail address)
 Copy of PRC/IBP ID (if professional)
 Copy of company ID
 Original PSA marriage contract (if traveling with spouse)

BUSINESSMEN:
 Copy of business registration from DTI or SEC
 Copy of business permit or mayor’s permit
 Original PSA marriage contract (if traveling with spouse)

STUDENTS:
 Original school certification
 Copy of school I.D.
 Original PSA birth certificate
 Parent’s document: Original bank certificate and bank statement or passbook for the last 3 months,
original COE/copy of business documents, and ITR

RETIREE:
 Copy of senior citizen or retirement documents
 Original PSA marriage contract (if traveling with spouse)

FOREIGN NATIONALS:
 Copy of I-Card

IF INVITED TO KOREA
 Invitation letter
 Copy of 1ST page of inviter’s passport or identification card (if personally invited by Korean)
 Copy of Korean company business permit (if invited by company in Korea)
 Additional documents may be required without prior notice

IF THE COMPANY WILL SHOULDER THE TRIP


 Guarantee letter from the company
 Original bank certificate of the company
 Copy of business documents and ITR of the company

FOR FOREIGN PASSPORT HOLDER: PHP 2,000 VISA FEE + PROCESSING FEE
VISA PROCESSING TIME: 7-15 WORKING DAYS (FIRST TIME TRAVELERS & FREQUENT TRAVELERS)

NOTE: ADDITIONAL REQUIREMENTS MAY BE REQUIRED UPON RELEASING OF PASSPORT.
