Handling of Hazardous Material I understand the safe handling procedures for the hazardous
materials

I understand that stationery vehicles should have their parking


brake applied and left in gear for precaution

I understand that if a Vehicle cannot be left in gear, wheel
chokes should be used front and rear
I understand that reverse parking is a must.
I understand that I must only drive a vehicle that iam
authorized and trained to use
I understand that I must maintain my assigned vehicle
everyday and prior to each use
Vehicle Driving
I understand that I must follow the highway code and respect
other motorist and predestrians

I understand that I MUST use signals at all times while driving.


I understand that its never acceptable for me to take shortcuts
ignore near misses or engage in horseplay

Safety Risk I understand that it is not acceptable to dispose waste in an


unauthorized facility or area and violation of this, is punishable
by law, and can lead to severe consequences
I understand that such activities as listed above will put me as
well as others at risk

Safety Hazards I understand that all safety hazards are to be reported and
addressed immediately
Accident Reporting I understand accident reporting procedures
I understand that Iam required to keep my work area and
vehicle clean
Work Zone Safety I will abide by good housekeeping practices
I understand that wandering around site is not allowed, I must
stay within the designated driver safety zone.

I have been inducted onto site and provided a copy of the Traffic Management Plan. I understand the
T&D Cleaners Site Rules, Traffic Site requirements and directions may result in my being refused entry to the
site. I agree to permit an authorized T&D Cleaners representative to view my logbook/work diary on request.
