What are my initial inferences from the referral question?
What is the basis/evidence for my initial inferences? What are my inferences now that I have reconceptualized the referral question? How do these inferences guide my interview of HPI and background history? (In addition to the usual areas to be covered) What are some clinically significant pieces of information in the history or in the HPI? Have I done additional exploration of clinically significant information? (history of current presence of anger, anxiety, depression, stress or distress, deaths, other kinds of losses, break ups, reaction to losses, coping, history of past trauma in case of personality assessment, etc.) What initial inferences or hypotheses have I been able to rule in our rule out while gathering the HPI? Which pieces of information in the history might be relevant for the referral question? What are the cultural considerations related to the problem? What additional inferences or hypothesis does this information lead me to? What are some notable observations in test taking behavior and attitude? What are some features of examinee that might be relevant to the current performance? (stability, emotional state, motivation, attention, concentration, focus, approach to problem solving, verbalizations?) What additional inferences or hypothesis does this observation lead me to? What is the main finding in each of the tests? Are the test findings within each of the test consistent or discrepant? How do I explain the discrepancies between findings within a test? What is the main finding in each of the tests? Are the test findings between the different tests consistent or discrepant? What are the repeated patterns among the findings of different tests? How relevant or significant are those findings? What are the discrepancies between the findings of different tests? How do I explain the discrepancies among findings of different tests? How does the history, current presentations and my observations fit with the findings? If yes, how do these various pieces of information present a coherent whole picture about the person and their performance? Is there any way where the history, current presentations and my observations go against my findings (inconsistent)? If so, how can I understand and explain these inconsistencies? What are the repeated patterns or themes that come up on the history, observation and test findings? How do these findings refute or confirm my hypothesis about what I think is going on? What does the emerging pattern tell me about this person’s _______________ (cognitive functioning, emotional functioning, personality make up, current mental/psychiatric state, or whatever is the purpose of testing)? How do the discrepancies fit the emerging picture? If the discrepancies do not fit the picture, what additional information or assessment is needed to explore this? How does this emerging coherent, whole picture explain the referral question? What clues does the current whole picture give us about what is missing or what needs to be done (recommendations)
In report writing
Have I read my reports several times to catch typos or editing errors?
Have I read me reports several times from the point of view of a lay person to see if it would make sense to a lay person? Is my report organized and structured? Does the language flow and is it easy to read? Do I explain the findings in clear, simple language that is understandable by a lay person? When using psychological jargon, have I explained and described it so that a lay person may understand it? Have I reported my findings in behavioral terms? Have I reported the relevance of the findings in everyday life of the examinee? Have I reported the findings in a way that answer the referral question? Are my recommendations consistent with the problem, in other words are the recommendations relevant and targeted to the problem identified? Are the recommendations specific, affordable and practical? Do my findings and summary directly address the referral question? Are my recommendations directly in response to the referral question? Have I signed off the report with my correct and appropriate credentials?
