FORMAT FOR THE COMMUNITY ORIENTED PRIMARY CARE REPORT

INTRODUCTION
1. Introduced the chosen population group and a health situationer of the said population
[ step 1 and 2 of COPC process]

INTERVENTION
2. Presented the health problems identified and that which was prioritized [ Step 3]
3. Presented the critical analysis of the prioritized problem that led to the choice of
strategies used. The analysis takes into consideration the presented health situation
mentioned above [Step 4]
4. Presented the SMART objectives [ Step 5]
5. Presented how the chosen strategies/ health promotion activities were carried out [
Step 5]

RESULTS AND DISCUSSION = EVALUATION


6. Presented the results of the intervention based on the objectives of the project [Step 6]
7. Presented both successes and limitations of the interventions, the resources used and
the barriers met during the intervention
8. Presented recommendations for future interventions
