Home Visitation Narrative Report

The Science Department at Surabay National High School conducted home visits to students who failed to submit their modules for the second quarter. They visited several barangays that had the highest numbers of students with missing assignments. The teachers found that financial issues due to the pandemic made it difficult for students to submit modules on time. Through the home visits, arrangements were made to help students avoid delays in submitting future assignments, allowing them to pass the quarter and continue their studies for the rest of the school year.

HOME VISITATION NARRATIVE REPORT

Purpose of the Activity:

Reaching out students who failed to submit their modules on the second Quarter.

Narrative;

The Surabay National High School Teachers of the Science Department headed by
______________conducted Home visitations to students who were not able to submit their modules for
the second quarter on ____________.

These includes Barangay ______________, ____________________, ______________, and


__________________ who has the highest number of students who might later encounter problem with
their grades due to non -submission of Modules.

List of target students from each curriculum to be visited were submitted to the Department upon LAC
Session.

Letters requesting permission were then sent to the office Punong Barangay.

Upon visitation, we concluded that financial incapacity is one of the raging reasons, especially now in
the time of Pandemic that makes it difficult for them to return modules on time.

Furthermore, agreements and arrangements were made to avoid the delay of return of Modules.

Generally, as we observe this activity, we have come up to the idea that the Home Visitation we made is
a success considering that the Students did not fail this Quarter and will continue with their studies for
the entire school year.
