PHOTOGRAPHIC RELEASE AND CONSENT AGREEMENT

I hereby willingly and intentionally grant to _________________________ (Photographer) the


immutable right and permission, in regards to the photographs that he/she has either taken or has
had taken of me, in which I may appear individually or with others, to copyright the same, in
his/her own name or otherwise (as well as assign my rights internationally in such photograph), to
use, re-use, publish, and re-publish, and otherwise reproduce, alter, edit, or modify and display
the same, in whole or part, individually or in conjunction with other photographs, and in
conjunction with any other copyrighted material, in any and all media now or hereafter known,
for illustration, promotion, art, advertising and trade, or any additional purpose otherwise
unmentioned; and to use my name in association with, if it so chooses. I hereby waive any legal
or assumed right to any and all claims and demands arising out of or in regards to the use of the
photographs, including, but not limited to, any and all claims for libel and/or invasion of privacy.

The aforementioned photographer may sell, assign, license or otherwise transfer all rights granted
to him/her hereunder without my knowledge or consent. I am eighteen years of age or older and
have the right to contract in my own name. I have read this agreement and fully understand the
contents of this “Release”. This release shall be binding upon me and my heirs, legal
representatives and assigns. Further, I release the aforementioned company from any
responsibility for injury or damage, physical or otherwise, incurred during the photography
session.

 I agree that the photographer may associate my name as a caption with the image(s)
created.
 I request that the photographer not associate my name with any image created.
