ECC Conditions
The proponent shall strictly implement the following mitigating, enhancement, and
rehabilitating measures:
1. That solid wastes generated during operation shall be properly handled, collected and
disposed in a sanitary manner so as not to cause nuisance pursuant to RA 9003 better
known as "Ecological Solid Waste Management Act";
The PCO is designated for the monitoring and management of all hospital wastes that ensures
proper segregation at source and transportation to storage area. The company is in partnership
with a private waste collector that transfers, process, treat, and dispose solid waste.
2. That a Water Discharge Permit and Permit to Operate (P/O) Air Pollution Control
Facilities (APCF) shall be secured from this Office prior to its operation and shall be
renewed thirty (30) days before expiration;
3. That pathogenic and infectious wastes and pharmaceuticals and drugs wastes
generated from hospital operation shall be subjected to appropriated disinfection
methods and practices so as to eradicate pathogenic bacteria which is detrimental to
public health welfare and safety, pursuant to the provisions of RA 6969 otherwise
known as the "Toxic Substances and Hazardous Nuclear Wastes Act of 1990"
The company ensures that all pathogenic and infectious wastes are properly disposed and
disinfected prior to transfer. Labeled-garbage are maintained and segregation or separation of
wastes are observed. Color-coded waste bags are supported and foreseen to be effective and
implemented around the hospital premises.
4. That the proponent shall comply with the provisions stipulated under the Clean
Water Act (RA 9275), Clean Air Act (RA 8749):
The PCO ensures that water sanitation are primarily monitored. Quarterly water analysis are
done. Cleaning of all faucets, drainage and tubes are done in timely manner. Genset is
checked with storage of diesel is reviewed.
5. That adequate drainage system, sewage treatment facilities such as septic tank.
retention ponds etc., and safe water supply system must be properly designed, installed
and maintained at all times of operations so as not to affect the quality of the receiving
body of water;
Waste water discharge permit is renewed and checked for the condition. Drainage system is
reviewed by EMB with company-based septic tank is checked. Safe water supply is ensured
and monitored through water analysis.
6. That safety and preventive measures, including the posting of warning signs in
conspicuous places of the project site shall be properly observed and practiced at all
times;
All signages are properly attached per area of direction. Warning signs are posted properly to
an area of applicability. All signages are visible and easily understood.
Before the reconstruction of the building, the ECC is secured with the management plan as
reviewed. The building is registered and obedient with the standard of credentials and
licenses.
8. That the EMB-12 reserves the right to cancel or withdraw this certificate for any
misrepresentation of facts vital to the issuance of this certificate and for any violation of
the conditions stipulated hereof;
The company ensures the processing of relevant data for all of the requirements of EMB are
concise and correct according to the data gathered by the PCO. The company secures
necessary intervention to promote a safe workplace and an environment for people coming in
and out of the hospital premises.
9. That in case of abandonment, the Proponent shall notify the EMB Regional Office
concerned within three (3) months prior to the abandonment and the Proponent shall
submit its abandonment mitigation plan;
The company accepts whatever the feedback evaluated by the EMB personnel. The PCO of
the company reports to the managing head for whatever the decision therein. The PCO must
ensure that a probability of appeal to the issuance of any cases set by the EMB must be
enforced.
10. The proponent shall employ a Pollution Control Officer (PCO) in accordance with
the Implementing Rules of RA 8749 particularly DAO 26, S. of 1991, who shall handle
the environmental aspects of the project, which shall have the following
responsibilities:
The PCO in charge monitors all pertaining responsibilities and duties. Pre-assessment and
post-assessment with in between inspections for all necessary guidelines to be followed. The
standard that is required to follow is assessed and evaluated to be compliant.
11. That a 2 x 4 billboard containing this message: “Notice to the Public. This project
Hospital Building Expansion of Auguis Clinic and Hospital has been issued an amended
Environmental Compliance Certificate No. (ECC-R12-1504-0037) from Old ECC Ref.
No. (ECC-R12-1008-0147) by the Environmental Management Bureau of the
Department of Environment and Natural Resources, Region XII, on APR 2 8 2015.”
This message must be installed at all entry and exit points and at all perimeters of the
project facing the road to inform the general public within thirty (30) days from receipt
of theertifyicate. Picture verification of compliance to the posting of notices and the
billboards shall be submitted to this Office within ninety (90) days from receipt of the
ECC.
Requirements for ECC before the reconstruction of the building was secured. ECC permit
was secured and all the documents submitted are true and correct.
12. That on-the-spot monitoring and inspection can be initiated by the EMB-12 anytime
to check compliance to the above stipulations;
The inspection of EMB is well appreciated by the company. This enables the company to
know which area of correction is needed to be addressed promptly. This identifies proper
checking of the compliance to the laws set forth by the DENR EMB. Monitoring and
inspection of the sites is much appreciated by the company. This maintains conciseness and
truthfulness of the documents and certificates granted.
13. The proponent shall immediately plant appropriate tree species within the project
area to compensate for the damaged vegetation:
Annual tree planting is done and has been observed for a couple of years since accreditation
to EMB was secured. All pertaining documents are submitted as proof of implementation.
Budget allocation for an annual tree planting has been approved.
IV. RESTRICTIONS
The proponent is strictly subject to the following restrictions:
14. That no other activities should be undertaken other than what was stipulated in the
IEE document. Should there be an expansion of the project beyond the project
description, construction of other structures beyond those stated in the IEE document;
or any change in the activity, shall be made subject to a new Environmental Impact
Assessment.
All expansion, reconstruction and other building works are arranged to the EMB personnel. It
is necessary that compliance of the management plan is followed. No reconstruction done not
known by the EMB personnel. No reconstruction done after the year 2017 building
reconstruction.
15. That land clearing shall be limited to the project area applied;
The proposed plan submitted at the EMB office is true and correct. Documents submitted
after the construction of new building and is seen and reviewed by the EB personnel for
correctness of data. ECC is established and maintain to be complaint to the laws.
16. That in case of transfer of ownership of this project, these same conditions and
restrictions shall apply and the transferee shall be required to notify the EMB Regional
Office concerned within fifteen (15) days as regards to the transfer of ownership.
No other ownership was made. The company is maintained to be DTI registered under the
single proprietorship owned by the owner since 1980. The project of the building is planned
by the owner alone.
