Dear, Dr. ____:

I, Juliana ___________, an applicant of BS in Computer Science, AY 2022-2023; humbly writing


this letter requesting from your good office to reconsider my admission to your esteemed institution,
the Polytechnic University of the Philippines, Sta. Mesa. In reference to my application, I was one of
the deemed qualified enrollees, however, my admission was declined due to the failure of my slot
confirmation within the annunciated date.

With utmost respect, I wish to introduce myself for reconsideration as a person of goodwill and moral
conduct. Recently, I am one of the successful awardees for the DOST-SEI Scholarship. Attached to
this letter shows the received NOA (Notice of Award) from DOST. Upon graduation, I aim to render
years of service as an educator to your respected university in fulfillment of my responsibility to the
country and your institution. You can also find here my Certificate of Academic Excellence presented
with High Honors. If given the opportunity once again, I am beyond enthusiastic and honored to be
another fellow Iska.

My failure of the slot confirmation was a lapse on my end. However, it does not fully define my
capabilities, and potential, not just as a student, but as a person. I was appointed to a position in life
for which I had little relish and privilege, yet full of plight. Nonetheless, just like any striving person
yearning for success, such as yourself, I also am inclined to refuse these preordained afflictions in life,
and fully seek the opposite fate of the cards I've been dealt with. A minute amount of support from an
ultimately few people is the only one I currently have to move forward. It is in my utmost sincerity
and respect to request you to reconsider being my biggest supporter yet.
