Branch 20 in Cebu will be approved. However my expectation failed as I received the dismissal
order instead, as attached. In connection to this, I would like to request your kind office to
change my family name in my personal data sheet, appointment and my other records from
MINESCU, MARIA DELIA G. TO SURBAN, MARIA DELIA M. as I have to retain my
married family name, to have consistency in all my government records such as GSIS,
Philhealth and PAG-IBIG.
