WORK IMMERSION SET-UP

Flow Chart of Work Immersion Communication and Supervision

STEP 1
Student must be officially enrolled in Work Immersion Subject

STEP 2
Student Trainee will submit his/her Study Load together with his/her Resume to the Work
Immersion Teacher

STEP 3
Work Immersion Teacher will release the Endorsement letter, Information Sheet & Waiver
Form.

STEP 4
Training Partner will send confirmation letter to the school c/o the Work Immersion Teacher.
The Teacher will release the Daily/Weekly Report Form & STAR to be accomplished by the
trainee.

STEP 5
Daily/Weekly Report is submitted weekly by the student trainee to the Work Immersion
Teacher.

STEP 6
Work Immersion Teacher will schedule a visit in the different training venues in order to
check the performance of the student trainees.

STEP 7
Training Partner and Work Immersion Teacher will schedule a meeting vis-à-vis Trialogue to
discuss issue and problems of the Training Program and Student Trainees.

STEP 8
Training Partner will release Training Certificate upon completion of the Training Period
assigned to the student trainee. He/ She presents to the Work Immersion Teacher his/her
“STAR” for verification. Work Immersion Evaluation form is released to the student trainee.

STEP 9
Work Immersion Evaluation is submitted to the Work Immersion Teacher. Oral presentation
date is set by the Work Immersion Teacher.

2. Application Letter addressed to the barangay Captain for work immersion.
3. 1-page Resume of the student trainee with 1x1 ID picture.
4. Endorsement Letter is the letter from the Work Immersion Teacher and noted by the
Dean endorsing the students for Work Immersion training to his/her chosen Work
Immersion venue.
5. Information Sheet contains all the necessary information about the student trainee. The
student trainee should fill up this form and be able to submit his/her Work Immersion
Teacher.
6. Student Trainee Waiver is signed by his/her parents/guardian. It states that they will
not file any civil, criminal or administrative case against Southwestern University and
the training venue for any injury that may be sustained by their son/daughter attributed
to his/her own negligence during his/her Work Immersion training must duly sign the
Student Trainee’s Waiver Form. It will also waive the school and the training venue
dorm the payment or medical expenses that may be incurred as a result of such incident.
7. Confirmation Form will be accomplished by the training venue supervisor confirming
the acceptance of the student trainee for training.
8. Daily/Weekly Report/ Narrative Report must be filled up by the student trainee. He/
She should report/write the things he/she learned during his/her training in a
particular department or area. Likewise, this daily/weekly report should be submitted
to his/her Work Immersion Teacher every week.
9. Student Trainee Attendance Record (STAR) is a record form that must be filled up by
the student trainee every time he reports to his workplace. This daily time record must
be duly signed by his/her supervisor to attest to his/her daily attendance. This is
submitted to the Work Immersion Teacher weekly during the Work Immersion class
meeting.
10. Training Certificate will be requested by the trainee from his/her Work Immersion
supervisor upon completion of his/her required number of Work Immersion hours.
11. Performance Evaluation Report will be accomplished by the student trainee’s
supervisor and will be submitted to the Work Immersion Teacher before the oral
presentation of the Work Immersion report.
12. Work Immersion Report is a major requirement to be submitted by the trainee, which
will contain all the needed documents and reports. Oral presentation will be scheduled
before the end of the semester.

POLICIES, RULES AND REGULATIONS

These are the following rules being implemented and followed by the interns.
1. Observe proper grooming.
2. Ladies should wear black close shoes with high heels.
3. Hair should be tied up.
4. Make-up should be light.
5. Ring of more than one in both hands is not allowed.
6. Earrings should be simple.
7. Always bring your DTR.
8. Interns must always log in on the logbook.
9. Confidentially should be always strictly observed.
10. Do not bring any company property with you outside unless otherwise mandated by
your supervising officer.
11. After duty, interns must log out in the logbook.

STUDENT TRAINEE’S ATTENDANCE RECORD (STAR)

INSTRUCTION: the student trainee will use the STAR for encoding regular attendance and
computing the number of hours he/she has already completed. Upon verification of the
accuracy of data (time in and out), the supervisor affixes his/her initial in the appropriate
column. The student submits the STAR to the Work Immersion Teacher together with the
daily/weekly report.

The following rules and regulations regarding attendance and punctuality must be observed
and implemented:
1. An excused absence (with letter from parents & medical certificate) is equivalent to a
2-day extension duty.
2. An unexcused absence is equivalent to a 3-day extension duty.
3. Two consecutive absences or more will require a conference with the supervisor and
Teacher. Sanction will be given after the conference.
4. Tardiness more than 15 minutes will be sanctioned with a 1-day extension duty.

Pursuant to Division Memorandum No. 213 series of 2021 dated April 6, 2021 which is
anchored on DepEd Memorandum No. Cl – 2020 – 0085 entitled “Guidelines for Work
Immersion Implementation During Crisis Situation”, the schools are tasked to design their own
class program. As stipulated in DepEd Order No. 39 series of 2018, the clarification and
additional information to DepEd Order No. 30 series of 2017, Work Immersion is a requirement
for TVL track while it is an optional for Academic, Arts and Design and Sports Tracks.

With this, JAPeR Memorial High School would like to request your good office to allow the
school to conduct a School-Based Work Immersion for TVL – Agri-Crop and Organic
Production on March 14, 2022 to April 8, 2022 and Community-Based Work Immersion for
General Academic Strand – ABM on March 19, 2022 to May 21, 2022 through operation of a
micro business enterprise in their barangay.

Rest assured that all our students will adhere to the IATF Health and Safety Protocols and
Parents waiver during the whole duration of the activity.
