Warmest greetings!

Pursuant to the approval and recommendation for progressive expansion by the President dated January 17,
2022, the Department of Education (DepEd) has released Office 00-OSEC 2022-003 or Interim Guidelines
on the expansion of limited face to face classes.

Since the Province of Pangasinan is now placed under Alert Level 1, Juan Facundo Elementary School
intends to participate in the implementation of progressive expansion of limited face-to-face classes. The
school has been assessed, validated and recommended by the SDO I Validation Team led by Schools
Division Superintendent Dr. Ely S. Ubaldo as compliant with the standards of the School Safety Assessment
Tool (SSAT).

In this regard, the undersigned respectfully request your good office through our Barangay Health Unit
(BHU) and Inter-Agency Task Force (IATF) to conduct an inspection of our school facilities, assess and
validate our compliance and readiness to conduct progressive expansion of limited face-to-face classes and
be granted Inspection Certification from Municipal IATF.

Your prompt attention to this request is highly appreciated. May you always stay in good shape to remain
strong in leading our town.
