1.0 INTRODUCTION
QSHE Limited has been contracted by KHNL to develop Operations Safety Case Report for the
Shagamu LPG Stockpoint.

As part of safe implementation of the scope of the project, HAZOP and HAZID sessions would be
carried out to identify and assess all credible major accident hazards associated with the operations
of the facility, to ensure implementation of adequate risk controls and mitigations for the all
identified risks. Recommendations from the HAZOP (if any) would be assumed implemented
during the HAZID session which would eventually be used to produce the Operational Safety Case.

2.0 DEFINITIONS

HAZOP (Hazard and Operability Study) is a systematic approach to determining potential


problems that may be uncovered by reviewing the safety of designs and revisiting existing
processes and operations in a process plant.

HAZID (Hazard Identification) is a technique for identification of potential hazards and threats. It
is the process of identifying what can go wrong in a facility operations and deciding the best
control and mitigation measures to manage the hazard. The major benefit of HAZID is that
identification and assessment of the critical safety hazards provides essential input to risk
management decisions. This will lead to safer and more cost-effective changes where applicable
being adopted with a minimum cost of change penalty.

3.0 WORKSHOP OBJECTIVES


The primary objective of KHNL Operations Safety Case is to demonstrate to the Nigerian
Midstream and Downstream Petroleum Regulatory Authority (NMDPRA) as the Regulator of the
O&G Downstream industry, the KHNL Management, Staff, and other stakeholders that essential
controls are in place, to effectively manage the Major Accident Hazards (MAH) associated with the
operation of KHNL facilities to a level As Low As Reasonably Practicable (ALARP).

Major accident hazards (MAH) are the hazards which have the potential to cause multiple fatalities,
extensive asset damage, massive environmental or socio-cultural effect, or which impact negatively
on the reputation of KHNL. It is a requirement of the Operations Safety Case that the threats of
MAH risks are analyzed and effective controls demonstrated.

4.0 SCOPE
The Workshop will focus on hazards associated with operation of KHNL and associated activities.

5.0 METHODOLOGY

5.1 HAZOP Methodology


HAZOP helps to identify potential hazards and operating problems on a process plant. HAZOP is
achieved by assembling multi-disciplined team and applying structured brainstorming technique
lead by an experienced leader.

A method of identifying potential hazards and operability problems in industrial plant. Generally
recognized as the foremost method for identifying potential hazards in complex plant that may
include new technology

5.2 HAZID Methodology


The HAZID session will commence with a grounding presentation by KHNL Personnel on the
facility description, objectives of the study and the boundary conditions. This will be followed by
questions and clarifications. The hazard identification and assessment will thereafter proceed
through brainstorming exercise.

The HAZID worksheet will be projected during the session to enable team members participate
efficiently.

5.3 Bow Tie Methodology


Bowtie is pictorial presentation of how a hazard can be hypothetically released and further
developed into a number of consequences. Bowtie is a tool used to represent the management of
risks associated with major hazards.

A bowtie consists of a top event, stating how a hazard can be released; a left hand side that
describes the threats and the barriers in place to prevent them from leading to the top event; and of
a right hand side that describes the potential consequences that may arise from the top event, and
the mitigation measures in place to prevent those consequences

The Bow Tie Analysis session will be carried out to further reduce the risks associated with the
Major Accident Hazards Identified. Bow Tie Analysis worksheet will be projected during the
session to enable team members participate efficiently.

6.0 GROUND RULES


The under listed Workshop rules shall be applicable during the entire session;
• Put all mobile phones on vibrate / silent mode.
• Side discussions divide and distract the house. It shall be avoided.
• Prior to Workshop session, it is expected that participants would have read and familiarised
with Workshop Terms of Reference.
• The quality of the Workshop is as good and reliable as the level of contribution by participants,
therefore your full participation and commitment is required.

• Brain-storming is an essential aspect of the Workshop session but issues that cannot be
immediately resolved would be parked so as to save time
• Workshop session is not meant for design review; hence all drawings must have been duly
approved by the relevant authorities with evidence to show for it.
• Adoption of any recommendation is at the discretion of the Facilitator
• The Workshop Facilitator reserves the right to moderate the timing for tea breaks/lunch and
individual contributions depending on the energy levels during the Workshop session.

7.0 ROLES AND RESPONSIBILITIES

Participants Responsibilities
All Participants Enforce ground rules, participate in the process but stay focused &
time sensitive.

To be present throughout the workshop unless previously agreed with the


facilitator (to avoid loss of input, back tracking, lack of ownership).

Facilitator Manage the execution of the workshop. Lead and direct the process. He
shall be responsible for efficiency of the review process, adequate
reporting of comments and subsequent follow-up of workshop
outcomes/actions for close-out.

Scribe Document the findings/recommendations of the workshop in a word


document/spreadsheet. The required information can also be captured in
a notebook and then transferred subsequently into SAFOP work sheet.
Issue draft report promptly.

Discipline/ Team Reps Be knowledgeable about existing facility and/or the scope of work for a
particular discipline in the project. Describe the existing facility or
discipline’s scope to the other participants. Answer questions relating to
the existing facility or applicable discipline.

8.0 RECORDING AND REPORTING

During the study, all foreseeable and credible hazards that are identified by the team are recorded
on a HAZID Worksheet. Risk matrix below, shall be used to rank the potential risk based on
likelihood and severity to decide if the existing protection is adequate.

At the end of the study the outcome will be analysed and all major accident hazards identified
would be subjected to Bow Tie analysis to further reduce the risks to ALARP.
