AFFIDAVIT

I, MARY DARLENE T. LLORIN, of legal age, Filipino, married, and a resident of


Anthea St. RJ Ville, Subdivision, Barangay Ibabang Dupay, Lucena City, Quezon,
Philippines, after having been duly sworn in accordance with law, hereby depose and
state:

1. I am the business owner of RDKK Frozen Food Trading located at MAGS


Building Purok Sariling Atin, Ibabang Dupay, Lucena City;

2. That the aforementioned business is concerned with the retail/wholesale of


frozen processed food products;

3. That the said business do not sell fresh or frozen meats other than frozen
processed food products;

4. That I am executing this Affidavit of Non-Selling of fresh or frozen meats to


support the processing of my renewal of business permit;

5. That I executed this affidavit in order to attest to the truth of the foregoing and for
whatever legal purpose this may serve.
