AFFIDAVIT OF NEAREST KIN

I, JOSELITO DIAZ PATALINGHUG, married, Filipino, of legal age, and


resident of Matumbo, Lapu-Lapu City, under oath, hereby deposes and states:
1. That I am the nearest kin/ next of kin of FLORENDA PATALINGHUG
VALENCIA, who died last January 21, 1960 at Lapu-Lapu City;

2. That the deceased FLORENDA PATALINGHUG VALENCIA was married


and had a child with her husband. However, both his husband and child are
both dead;

3. That the deceased FLORENDA PATALINGHUG VALENCIA is my aunt;

4. That this affidavit is made in support of the undersigned’s request to obtain a


copy of the Death Certificate of Florenda Patalinghug Valencia at the Local
Civil Registrar of Lapu-Lapu City and/or the Philippine Statistics Authority;

5. That the Death Certificate is one of the requirements for the processing of
the estate tax of the estate of the deceased with the BIR;

6. I am executing this affidavit to attest to the truthfulness and veracity of the


foregoing statement and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 28th day of


September 2021 in Cebu City, Philippines.
