ACCREDITATION REQUIREMENTS VALIDITY


Commercial Layer Farm - Negative Avian influenza (AI)Laboratory Test Result 6 months
(Table Eggs) - Filled up Poultry Farm Profile (Signed by LGU or RFO Veterinarian)
- Vaccination and medication program
- Pictures of Biosecurity
- Negative AI laboratory test result ONLY 2 months
*(A 6-month-valid certificate will be issued upon submission of Filled up Poultry Farm Profile (Signed by LGU or
RFO Veterinarian), Vaccination and medication program, and pictures of Biosecurity).
For clustering (No individual farm laboratory results) 2 months
- Should belong to a cluster (barangay) wherein at least one (1) farm has been tested for AI
- Mayor’s permit or any document validating the existence/operation of the farm and farm address
*(A 6-month-valid certificate will be issued upon submission of negative laboratory result for AI, filled up Poultry
Farm Profile (Signed by LGU or RFO Veterinarian), Vaccination and medication program and pictures of
Biosecurity)

REQUIREMENTS FOR ISSUANCE OF LOCAL TRANSPORT/SHIPPING PERMIT

REQUIREMENTS
Table Eggs - BAI Certificate of Poultry Farm accreditation or certificate of registration/accreditation of source farm by the LGU Vet.
- Livestock Handler’s License
- Registration of Transport Carrier
- Avian Influenza-free Certificate attesting that:
1. the eggs were produced and packed in an avian influenza free area
2. the eggs were transported in new or appropriately sanitized containers;

- BAI Certificate of Poultry Farm Accreditation


- OR: (1)Certificate of ND vaccination issued by a licensed veterinarian indicating the types of vaccination and type of vaccine
Gamefowls used (Shipping permit shall be issued two (2) weeks from the date of vaccination; and (2) Negative AI laboratory test result
prior to shipment ( Test result shall be valid for one (1) shipment only)

- BAI Certificate of Hatchery Facility accreditation


- BAI Certificate of Poultry Farm Accreditation
- Veterinary Health Certificate (VHC)* signed by the farm veterinarian and concurred by a government veterinarian.
*For DOCs/HEs : The VHC indicates that the source farm has NO reported cases of ND during the last 21 days, the source flock
were vaccinated against ND (vaccination program, indicate type of vaccine used) and that the DOCs were vaccinated against
ND at the hatchery).
*For RTL Pullets: The VHC indicates NO reported cases of ND during the last 21 days, and the pullets were vaccinated against
Hatching Eggs/
ND within the last three (3) months (vaccination program, indicate type of vaccine used)

- For DOCs/RTL Pullets: AI-free Certificate attesting that the birds showed no clinical sign of AI on the day of shipment; the birds
were kept in AI free area at least for the past 21 days; and the birds were transported in new or appropriately sanitized
containers.

- For fresh quail eggs : AI-free Certificate attesting that the eggs were produced and packed in an avian influenza free area AND

kept in AI free area at least for the past 21 days; and the birds were transported in new or appropriately sanitized containers.
Ducks for Meat and - BAI Certificate of Duck Farm Accreditation

Embryonated Eggs/ - For live ducks: AI-free Certificate attesting that the birds showed no clinical sign of AI on the day of shipment; the birds were
Balut kept in AI free area at least for the past 21 days; and the birds were transported in new or appropriately sanitized containers.

Grazing Ducks - Two (2) negative AI Laboratory Test results (2 weeks apart)

- AI-free Certificate attesting that the birds showed no clinical sign of AI on the day of shipment; the birds were kept in AI free
area at least for the past 21 days; and the birds were transported in new or appropriately sanitized containers.
* Test results shall cover the entire flock population when the sample collection was done and shall be valid for one shipment only.
Racing Pigeons - Certificate of BAI accreditation of racing pigeon association
- Livestock Handler’s License
- Registration of Transport Carrier
- AI-free Certificate attesting that the birds showed no clinical sign of AI on the day of shipment; the birds were kept in AI free
area at least for the past 21 days; and the birds were transported in new or appropriately sanitized containers.

- VHC signed by the farm veterinarian and concurred by a government veterinarian attesting that the source farm has no
reported cases of ND during the last 21 days, and the chickens were vaccinated at least twice against ND (indicate the dates of
vaccination and type of vaccine used)
- Must be transported directly from the farm to the poultry dressing plant.

b. any equivalent treatment which has been demonstrated to inactivate avian influenza virus – the manure are completely
dried
3. the necessary precautions were taken to avoid contact of the commodity with any source of avian influenza virus

The VHC and AI-free Certificate shall be required prior to issuance of Shipping Permit.
The AI-free certificate shall attest that the live birds/poultry products were sourced outside of the 7 km radius control area.

Shipping Permit shall be issued by the nearest Veterinary Quarantine Office or BAI- deputized Veterinary Quarantine Officer.
ADDITIONAL REQUIREMENTS FOR ISSUANCE OF LOCAL TRANSPORT/SHIPPING PERMIT WITHIN THE 7 KM. CONTROL AREAS

Broiler - Negative AI Laboratory Test Result before harvest (Test results shall be valid for the entire cycle)
*The birds shall be slaughtered only in NMIS-accredited dressing plants.

Table Eggs - Negative AI Laboratory Test Result of source farm – valid for 21 days

Hatching Eggs - Negative AI Laboratory Test Result – valid for 21 days


- Sourced from farms without unusual mortalities
- Transported in a disinfected and sealed chick van before leaving the controlled area
- Fumigated at the source and upon arrival at the hatchery

Embryonated Eggs (Balut) - Negative AI Laboratory Test Result of source farm/s – valid for 21 days

REQUIREMENTS FOR POULTRY MEAT, POULTRY PRODUCTS

A. For poultry meat, including but not limited to fresh, frozen meat:
Shipping Permit shall no longer be a requirement.
Meat from domestic materials: shall be accompanied by a Meat Inspection Certificate (MIC)issued by the NMIS Meat Inspection
Officers.
Meat from imported materials: shall be accompanied by a Certificate of Meat Inspection (COMI) of Imported Meat and Meat Products
for Domestic Transport, issued by the NMIS Meat Inspection Officers.

B. For processed poultry meat and poultry products (BOTH uncooked AND fully-cooked):
Shipping Permit shall no longer be a requirement.
Manufacturers and distributors of these products shall only secure the License to Operate (LTO) of the meat processing plants or
Certificate of Product Registration (CPR) issued by FDA.
If no LTO and CPR from FDA, the LTO issued by NMIS which is still valid shall be honored.

C. For egg products (including but not limited to liquid egg, powdered egg)
Shipping Permit shall no longer be a requirement.
Manufacturers and distributors of these products shall only secure the LTO or CPR from FDA.
