STUDY HABITS QUESTIONNAIRE

Directions: Think of your current study habits. Read each statement below and circle “YES” if
the statement matches your current study habits or “NO” if the statement does not match your
current study habits. Once you have answered all 10, add up the total “YES” responses and the
total “NO” responses and put your totals on the lines provided at the bottom of the questionnaire.
