AFFIDAVIT

I, LUZVIMINDA DAYAG QUEREZA, Filipino, of legal age, married, and a


resident of Calamagui, Amulung, Cagayan, Philippines, after being duly sworn in accordance
with law, depose and state:

1. That I am one of the surviving legal heirs of the deceased HERMINO DAYAG who died
intestate leaving an unregistered parcel of land described as Lot No. 2914-G containing
an area of TWO HUNDRED NINETY FIVE (295) square meters located at Brgy.
Calamagui, Amulung, Cagayan;

2. That my deceased father is the Survey Claimant of the said unregistered land as per
CENRO, DENR;

3. As such, I am requesting the Office of the CENRO, DENR not to entertain any
transaction involving the said subject lot in order to protect my rights and interest over
the said lot.

IN WITNESS WHEREOF, I have hereunto set my hand this 13 th day of February 2023 at
