1 TIME, DATE AND PLACE OF INCIDENT: On or about 13 September 2022 around 1:15 in the
afternoon, in front of Felix Balgos National High School located in Agbalo, Panay, Capiz.

2 VICTIM: Edmar y Barradas, 22 years old, married, and a resident of Agbalo, Panay, Capiz.

3 SUSPECT: Two (2) unidentified persons.

4 BRIEF FACTS OF THE CASE: On the aforementioned TDPI, victim was shot to death while
walking along the national highway by two (2) unidentified persons riding a Mio FI125, color
yellow, unregistered plate number. The suspects are wearing a grey T-shirt and black pants,
with medium build. The victim sustained gunshot wounds on his torso and head while the
two (2) unidentified suspects fled after the commission of crime.

1 REFERENCE: Spot report on Murder Case dated 13 September 2022. Request be

informed that the suspect was identified as Jose Manuel Bulaclac, 28 years old, married,
unemployed, and a resident of Tico, Panay, Capiz. The suspect was arrested at his house
during a raid operation conducted, but the weapon used in the commission of the crime
was not recovered and he is now in custody for proper disposition. The criminal
complaint for murder was prepared for filing against the suspect at prosecutor’s office.

Joshua Pacio and reported
that a shooting incident
transpired at the Barangay
Hall of Victory, Abuyog,
Leyte. Upon receipt of the
report, the team led by
COP proceeded at the said
place and upon arrival
thereat, said team together
with the said Brgy Tanod
saw the victim lying with
his blood identified as
Jomel Bautista, 30 years
old, Jeepney driver and a
resident at Brgy. Victory,
Abuyog, Leyte while the
suspect is identified as
SPO1
JOHN PATRICIO, 36 yrs
old, married, presently
assigned in Hilongos
Police
Station, resident at Brgy,
Victory Abuyog Leyte.
Initial Investigation
disclosed
that SPO1 PATRICIO
went to the house of
Jomel Bautista to advise
him to stop
the loud music of karaoke
but Bautista and his
companion did not pay
attention
that resulted to a
misunderstanding wherein
SPO1 Patricio said that he
will
report to the barangay but
SPO1 Patricio said this
statement “ HINDI AKO
NATATAKOT SA
BARANGGAY PULIS
AKO” thus Jomel Bautista
went to barangay
hall without knowing that
SPO1 Patricio followed
him and shot him in his
head
that caused his instant
death. SPO1 Patricio were
arrested and recovered
from
his direct possession and
control is one (1)
magazine loaded with 12
live ammos.
The suspect was arrested
and brought at Abuyog
Police Station together
with
the recovered items for
proper disposition.

this station that there is a shooting incident occurred at the front of Felix Balgos National High
School on or about 1:15 in the afternoon 13 September 2022.

Entry Nr. 13-2622/2 Sep 13/1:35PM: Person of ALFONSO y VILLIARIZE appeared at this office
and reported that the victim who was shot that led to murder was EDMAR y BARRADAS.

AFFIDAVIT OF WITNESS

I, Eduardo Baldisimo, 50 years old, married, and a resident of Cabugao Oste, Panay, Capiz,
after having sworn in accordance with law, do hereby depose and state:

1. I have worked as a security at Felix Balgos National High school for four (4) years, since 2018.
2. I do not personally know the deceased, but I always see him because he is a tricycle driver
brought by the students.
3. There was many reports and complaints about the deceased arrogance and rudeness.
4. On September 13, 2022, the victim while waiting for a passenger student and a teacher
around 1:10 in the afternoon, a person passed by on a yellow motorcycle with a mask on his
face, nothing and suddenly fired an unidentified gun and at the same time as the shot, the
victim felt down where he was standing, and the suspect quickly run away. As a security
guard, I quickly went to victim to help him, but it was covered by on his blood and conscious
and I quickly called the Police Station about the shooting incidence.

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the filing of
criminal Case against Jose Manuel Bulaclac for violations of murder.

in accordance with law, do hereby depose and state:

1. That I was present and personally saw that the accused is present in the crime scene. That
he committed the crime of murder against the victim Edmar Barradas.
2. That we saw the victim lying unresponsively in the place of the commission of the crime.
3. That at about 1:10 PM of 13 September 2022, me and my son is heading home when we saw
the actual shooting of the victim by the accused.
4. On which on the same state, 2 men fled from the crime scene riding a motor.

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the filing of
criminal Case against Jose Manuel Bulaclac for violations of murder.

II. FACTS OF THE CASE

6. That on September 13, 2022, at about 1:15 pm, this office received information that
one Edmar Barradas was shot by an unidentified suspect in front of Felix Balgos
National High School at Agbalo, Panay, Capiz.

7. SOCO Team recovered evidence from the scene. The team conducted a deep
investigation and found the primary suspect. With that Preliminary Investigation,
Questioning and Interrogation of the suspect has been done. Though Questioning
and Interrogation suspect admit his guilt. And found out using illegal firearm.

8. The suspect was one Jose Manuel y Bulaclac, 28 years old, married, unemployed, a
resident of Tico, Panay, Capiz. The crime happened due to personal issues and
intensions.

9. A case for illegal Possession of Firearm under Arms Act A case for Murder under
Revised Penal Code (RPC) of Article 248.

Initial investigation conducted by the responding police officers, the victim was identified as
Edmar Barradas, 22 years old, married, a resident of Agabalo, Panay, Capiz. Accordingly, the victim
was waiting for the passenger when the suspect shot the victim 2 times while riding a Mio FI125,
color yellow, unregistered plate number together with his accomplice and fled after the commission
of crime. The suspect was identified as Jose Manuel Bulaclac, 28 years old, married, a resident of
Tico, Panay, Capiz.

The undersigned, Chief of Police of Panay Police Station, accuses Jose Manuel Bulaclac, In
the municipality of Panay, province of Capiz, Philippines, the said accused did then and there,
willfully, unlawfully, and feloniously, with malice and aforethought, attacked by 45-gun pistol with
intent to kill, producing wounds which are necessarily fatal, thereby causing the immediate death of
Edmar Barradas.
