This affidavit of loss was sworn by Noel M. Pandac, an employee of Mundo Builders Inc. Pandac states that on February 9, 2023 while going home, he discovered that his employee ID issued by Solaire North Project had fallen out of his bag. AFFIDAVIT OF LOSS

I, NOEL M. PANDAC, of legal age, married, and with residence at 96 Kaliraya St.,
Brgy.Tatalon, Quezon City, Philippines, after being duly sworn to in accordance with law,
hereby depose and state that:

1. I am an employee of Mundo Builders Inc., as such I was issued an Employee ID.

2. On February 9, 2023, on my way home, I discovered that my ID in Solaire North Project


has fallen off in my bag.

3. That despite diligent search and effort to locate the above-mentioned Employee ID, I
could not find the same such that I now believe that it is now lost beyond recovery;

4. I execute this affidavit to attest to the truth of the foregoing, and to secure another
employee ID, and for whatever legal purpose it may serve.
