1. PURPOSE
The purpose of Organization Chart is to give information related the person who will carry out
the work, and give clear information the person in charge as each section.

2. SCOPE
Scope of work of construction package is Engineering, Procurement and Construction for Pro
Tank Expansion Project Phase 3.

3. DEFINITIONS AND ABBREVIATIONS


PROJECT : Pro Tank Expansion Project Phase 3
OWNER : PT. Pro Tank Terminal
Contractor : PT. Sarana Remaja Mandiri

4. JOB DESCRIPTION
This will describe the job description for each position in the project organization.

4.1. Project Manager


 Responsibility to execute the work in timely manner.
 Responsibility to deliver the project as per Contractor Requirement & Specification.
 Manage, lead & control for whole project organization.
 Coordinate with Contractor Representative to achieve project goal.
 Accountable for the implementation of HSE management system on the project site.
 Control the project budget.
 Attend Monthly Meeting with the Contractor Representative.
 Lead project Change Management.

4.2. Project Admin

 Report to project manager


 Manage document from client & contractor
 Distribute document from client & contractor
 Update document form client & contractor which include information regarding tittles,
numbers, revisions, and progress status from start to finish

4.3. Engineering

4.3.1. Engineering Manager

 Report to Project Manager


 Primary responsibility to lead, manage, coordinate, and control engineering efforts to
support the procurement, construction, QA/QC and pre-commissioning activities as well as
compliance to any OWNER’s Technical Requirements.
 Responsible to define the Scope of Work of the design and the preparation of the design
in line with the specifications and requirements. Responsible for the review of the design
by the Engineer and authorization of Design Reports and Drawings.
 Accountable for project’s facilities engineering definition, completion and accuracy
throughout the detail designs, procurement phase and providing resolutions on any
engineering issues during construction and pre-commissioning and as-built deliverables.

 Manage, coordinate and ensure alignment of engineering efforts with the vendor’s design
activities, Operation and Maintenance requirements.

4.3.2. Engineer

 Report to the Engineering Manager.


 Prepare Detail Design Data, calculations, data sheets, construction specification, material
specification, etc.
 Review Material Requisition and technical part of the subcontract inquiry documents.
 Check and put precise and understandable comment to vendor documents and return to
vendor as early as possible.
 Quality control and schedule control on his assigned activities.
 Resolving incomplete, ambiguous or conflicting item in output/input data among
disciplines, project team, client and/or vendor.
 Prepare TQ/change request.
 Supervision and coordination with CAD drafters.

4.3.3. Drafter

 Report to the Engineer.


 Development of design drawings by using CAD in accordance with their function and
objective as directed by the Engineer.

4.3.4. Document Controller

 Report to the Engineering Manager


 Managing all commercial and technical documents for the Owner. Implement processes
and tools, providing system reports, maintaining and updating electronic information
systems, distributing documents, and ensuring accuracy of all documentation are all
essential functions of a document controller.
 Establish and update the Document List (for outgoing engineering document and
incoming/outgoing Vendor deliverables)
 Allocate the engineering documents identification numbers.
 Manage the Document Control Center.
 Perform internal distribution of other technical documentation as required by
 Project Management.

4.4. Project Control

4.4.1. Project Control Manager

 Reports to the Project Manager


 Responsible for the project scheduling including establishment of the Project Master
Schedule and other planning document in coordination with the various teams of the
organization.
 Responsible for the project cost control in coordination with the various other parts of the
organization.
 To report and compare actual project performance against the project management plan.
 Collecting and distributing performance information, including status reports, quantity
surveying, progress measurements, and forecasts.
 Manage Deviation Notice and Change Order Process.
 Chair Weekly and Monthly Project Review Meetings.

 Participate in Monthly Project Inspection and Audit.

4.4.2. Planning & Scheduling Staff

 Reports to the Project Control Manager.


 Develop, update, monitor and report Project Master Schedule.
 Providing information to support status schedule reporting, progress measurement, and
progress forecasting.
 Determining the current status of project schedule.
 Providing forecasts to update current schedule information.

4.4.3. Cost & Billing Staff


 Reports to the Project Control Manager.
 Providing information to support status of cost reporting, progress measurement, and cost
forecasting.
 Determining the current status of project cost against physical progress and planned
schedule.
 Providing cost forecasts per planned activities.
 Prepare monthly progress measurement and collect evidence for billing purpose.

4.4.4. Procurement Staff

 Reports to the Project Control Manager.


 Develop the Tender documents together with Engineering, QA/QC teams including
but not limited to Material Take Off (MTO), Specifications, Data Sheet, Terms and
Conditions, Tender Schedule, Delivery Time, Delivery Term, vendor assistance
requirement, Documentation requirements etc.
 Issue inquiry to vendor with clear acknowledgement from vendor including but not limited
to quote or not quote.
 Receive the vendor proposals with proper documentation control.
 Coordinate Engineering, QA/QC teams for any clarification meeting with vendors and
properly document all the technical clarifications minutes of meetings.
 Arrange preliminary negotiation with prospective vendors.
 Prepare weekly and monthly report on the Purchase Order issued under his jurisdiction.
 Organize Kick off Meeting with vendor.
 Commence expediting activities as per Kick Off Meeting result.
 Coordinate all Expediting activities with including but not limited to monitor the fabrication
progress, sub vendors progress, vendor print documents, delivery schedule, packing and
marking, handling procedure, preservation procedure and shipping procedure.
 Record all correspondence, communication, monitoring, periodically and progress report
from vendors.
 Issue the Delivery Completion Notice (DCN) for any completed delivery.

4.4.5. Material Control Staff

 Reports to the Project Control Manager.


 Manage logistics activities for materials including but not limited to receiving,
inspecting, unloading, storing, maintaining, protecting, preserving, issuing, recording,
inventory and handing over with all administration works.
 Report for the logistics of all temporary materials, tools and consumables, including but
not limited to receiving, inspecting, storing, protecting, issuing and balancing within the
minimum stock requirement for the construction purposes.

 Coordinate with all related parties such as Procurement Staff, QA/QC, HES, for
all material management activities in order to achieve material, equipment and
consumable supply to the project in a timely manner.
 Review and Check the Material Balance Status Report before issuance to
Project Management team including but not limited to:
o Status of material receiving
o Status of material issuing
o Status of material balancing
o Inventory Report as per current status

4.5. QA/QC

4.5.1. QA/QC Lead


 Reports to the Project Manager.
 Develop QA/QC Plan.
 Responsible to implement and control the provisions of the Quality System as per this PQP.
 Coordinate with Corporate QA/QC department for developing WPS/PQR.
 He is responsible to ensure inspection of all field production activities in accordance
with the Inspection and Test Plan (ITP) are carried out and well documented.
 Coordinate with Document Control for Handover of Final Documentation.
 He shall coordinate for initiation of corrective measure identified in non- conformance
reports until properly closed out.
 He shall lead the QA/QC Inspectors for test and certification of crafts such as welders,
including coordination with GOI Certification Agencies.
 Responsible for provide QA/QC Monthly report & performance.

4.5.2. QA/QC Inspector/Surveyor

 Reports to QA/QC Lead


 Conduct day to day inspection activities for their assigned discipline areas and guide
Construction Supervisors on ITP requirements.
 Witness and sign off ITP site tests and inspection.
 Inspect, accept and close out all non-conformances.
 Prepare QA/QC Report for each inspection as per ITP.

4.6. Construction

4.6.1. Site/Construction Manager


 Reports to the Project Manager.
 Primary responsibility to lead, manage, coordinate, and control construction activities
as well as compliance to any Owner's Requirements.
 Prepare constructability plan.
 Responsible for construction progress.
 Ensure construction schedule follow the Project Master Schedule.
 Coordinate with PPC department to analyze project forecast.
 Coordinate with PPC department to develop manpower & equipment planning for
construction activity.
 Responsibility to lead, manage, coordinate, and control construction supervisor & foreman.
 Attend Weekly Coordination Meeting and Monthly Meeting.

4.6.2. Construction Supervisor


 Report to Site/Construction Manager.

 Review and countercheck construction drawings received are latest revision and
 properly stamped “FOR CONSTRUCTION”.
 Based on priorities within his area of responsibilities, plan which areas will be divided into
the number of Foremen under his supervision and discuss the specific scope of works with
the Foremen.
 Prepare all the needed tools, equipment, consumables, PPE, vehicles; HES
 support materials and manpower needed to start with the works.
 Prepares specific SOP & JSA needed for application of Work Permit. He shall also be
responsible to obtain, extend and close the Work Permits.
 Ensures that all labour under his care are properly oriented, equipped with proper
 PPE, have valid badge and fit for work.
 Supervise and ensure construction activity meet safety requirement.
 Coordinate with QA/QC on inspection and tests needed as per ITP.
 Lead or attend Pre-Job Tool Box Meetings within his jurisdiction

4.6.3. Welder Supervisor

 Report to Site/Construction Manager.


 Responsible to manage welder group & foreman.
 Based on priorities within his area of responsibilities, plan which areas will be divided into
the number of Foremen under his supervision and discuss the specific scope of works with
the Foremen.
 Prepare all the needed tools, equipment, consumables, PPE, vehicles; HES support
materials and manpower needed to start with the works.
 Ensures that all labour under his care are properly oriented, equipped with proper PPE,
have valid badge and fit for work.
 Supervise and ensure construction activity meet safety requirement.

4.6.4. RESS Supervisor


 Report to Site/Construction Manager.
 Responsible to manage Rigging group, Equipment/Heavy Equipment, Scaffolding team &
mechanic for maintenance the Equipment/Heavy Equipment.
 Based on priorities within his area of responsibilities, plan which areas will be divided into
the number of Foremen under his supervision and discuss the specific scope of works with
the Foremen.
 Prepare all the needed tools, equipment, consumables, PPE, vehicles; HES support
materials and manpower needed to start with the works.
 Ensures that all labour under his care are properly oriented, equipped with proper PPE,
have valid badge and fit for work
 Supervise and ensure construction activity meet safety requirement.

4.7. HSE

4.7.1. HSE Lead

 Accountable for the implementation of HSE management system on the project site.
 Develop HSE Plan.
 Coordinate/communicate with Construction & other section regarding the
implementation of HSE Plan.
 Ensure all corrective actions raised to all gaps and weakness found during HSE
 inspections and/or audits are being implemented.
 Ensure all rules and regulations related to HSE are regularly reviewed and updated.
 Ensure all project activities are conducted in a safe manner.

 Establish and maintain regular interface discussion with Contractor HSE


 representative.
 Develop HSE performance statistic at the workplace.
 Develop training and education program for all workers at project site.
 Prepare project regular report (monthly report, HSE project overview).

4.7.2. Safety Officer/Inspector

 Inspect all scaffolding, construction electricity system, heavy equipment,


machineries, tools, vehicles, and other equipment will be used in project as per
competency.
 Develop an inspection report and communicate it with HSE Lead.
 Conduct regular inspections at infrastructure facility area.
 Has a qualification or certified as Inspector for each respective area. E.g. Inspector for
scaffolding shall be a qualified or certified Scaffolding Inspector.
 Conduct daily safety patrol to ensure the work are execute in a safe manner.
 Assist HSE Lead to provide HSE performance statistic & report.

4.7.3. Paramedic
 Respond immediately and deliver initial stabilization for injured / illness person at the
scene prior transfer to site clinic.
 Implementation of health management programs.

4.8. HC & Finance

4.8.1. Lead HC & Finance


 Responsible to handle finance and human capital at site.
 Responsible to provide site accommodation and office facility.
 Handle for manpower site hiring.
 Perform financial and accounting functions on the site.
 Carry out general affairs functions.

4.8.2. Finance

 Assist Lead HC & Finance to carry out finance functions.

4.8.3. General Affair


 Assist Lead HC & Finance to carry out general affairs functions.
 Deal with transportation for labour.
 Handle site accommodation and office facility.
