We, ____________________, of legal age, Filipino, and a resident of Davao City,


Philippines, ____________________, of legal age, Filipino, and a resident of Davao City,
Philippines, respectively, after being duly sworn to in accordance with law, do hereby depose
and state:

1. That we personally know JANICE of Davao City, Philippines;

2. That “JANICE” and “JANEES ” is the same person;

3. That we are executing this Joint affidavit of Two (2) Witnesses to attest to the truth of
the foregoing facts, and for whatever legal purpose it may serve;
