I, DIANNE MICHELLE GUTIERREZ CAYABYAB, of legal age, single, Filipino Citizen and
with residence and postal address at Block 1 Lot 12 Greentown Villas - 1, Mambog 3, Bacoor, Cavite
City, herein represented by, DAVES LAXAMANA CAYABYAB , of legal age, married, Filipino
Citizen and with residence and postal address at Block 1 Lot 12 Greentown Villas - 1, Mambog 3,
Bacoor, Cavite City, as my attorney in fact, under oath state:

1. That I am one of the Vendee/Buyer of a certain parcel of land Covered by Transfer


Certificate of Title No/s. 057-2019008806 situated at Block 3, Lot 19 Montefaro Village
West Brgy. Alapan 1-B, City of Imus, Province of Cavite, Island of Luzon;

2. That said document was notarized by Atty. Gilberto B. Pasimanero., last January 13, 2023.
With Doc. No. 900, Page No. 180, Book No. I, CM Series of 2023;

3. That under said Deed of Absolute Sale, the civil status and age of consent was not stated;

4. That the latter should be stated and indicated as:

“DIANNE MICHELLE GUTIERREZ CAYABYAB, of legal age, single, Filipino Citizen and
with residence and postal address at Block 1 Lot 12 Greentown Villas - 1, Mambog 3, Bacoor, Cavite
City.”

5. That I am executing this Affidavit to attest to the truth of the foregoing and for all the other
legal purposes this may serve.
